My daughter started care the 1st week of October I have never seen this form beforeThere are many additions that were not in place when we started in early October, such as sign in and sign out sheetThat sheet did not appear at cubbies until the week prior to us leaving and one of MsP [redacted] s's workers (Vailia) told us it was "state law to complete and should have been there all along." I replied I was happy to completeI could continue on with other additions made after we enrolled that I was not aware of I gave notice on 12/14/that we would no longer be needing care effective 1/19/That is more than weeksThere were days my daughter missed the bus home from school and I texted immediately that she would not be there for aftercare as I had to leave work and pick her upMsP [redacted] was notified promptly that my child would not be thereShe was paid in full for both daysI also gave a full weeks notice that my daughter would not be there from 11/15/17-11/29/as we were out of town on holidayAgain, she was paid in full for these weeks I have multiple emails from MsP [redacted] indicating my daughter was participating and happy there and that they "loved US." Was very shocked to hear my child and I was causing so many issues AFTER our care was immediately terminatedRegarding not paying late fees, I gave late fees personally to both MsP [redacted] 's sister as well as Viala and have email receipts as well as text receipts from MsP [redacted] that they were receivedI do not know who MrP [redacted] is...assuming it is her husband, or her father, but I never met either of themI understand MsP [redacted] saying there is no refund on $deposit, as the time needs to be usedWhen I gave my notice on 12/14, we had worked out the month of January being used in leiu of payment (utilizing the deposit)On 12/18/I had a conversation with MsP [redacted] at pick up advising we would not be at care the next weeks, as school was on break and I did not have anyone to take my daughter there from 2:30-5pm, as I was working all dayShe said that was fineI never said we were not coming back, nor were we asked thatI also never said we were not going to pay for those weeks, merely, we would not be thereWe had communicated the week before about our last day being 1/18/Nothing had changed and we left MsP [redacted] 's houseI then get a text that we are being terminated effective immediately and not welcome backThis was Monday eveningI had paid $for that weeks care the Friday priorOne day of care was used ($30) and that money was not returned either Again, all I am asking is for my money backI have not seen the attachment provided beforePaperwork I have from MsP [redacted] include the welcome packet, a water/pool safety form, a sunscreen form, a media release, emergency contact form, family registration, parental liability for transportation, a general health form and a photo release I would like to resolve this as timely as possible and appreciate any assistanceMy daughter remains upset that she was not allowed back here as she really enjoyed her time here as well as the caregiversShe has special needs and I do not have the heart to tell her the reason MsP [redacted] is saying she cannot come back was in part because of my daughter's behaviorI again find this odd because MsP [redacted] asked if we wanted to stay late days a week to pair up with another child who was there because the two children "got along so well and my daughter played so well with this child." Late fees were never being paid. She always complained about them and as immediate termination, my policy says routinely late picking up your child. I offered her two weeks to come back in and she refused. In my contract, it states no refund on deposit, they can only come in for 2 weeks.

I would like to resolve this as timely as possible and appreciate any assistance. I terminated the agreement immediately because I felt threatened as well as my family feeling threatened. There is no refund on the deposit if there is immediate termination and that occurred because of her ex-husband coming to my house screaming on the top of his Lungs and trying to fight my...
