Per Mr**'s insurance policy with *** he is responsible for his co-pay and any labs or procedures performed until he has met his deductible. His daughter was seen August 29, and his plan year began August 1, 2017. KidMed has contacted *** to confirm the billing codes
used are the codes *** requires Kidmed to use. The amount Mr** stated he was billed for is significantly higher then his actual billWe have requested *** send him another explanation of benefits and we will send another copy of the bill
