included [redacted] and [redacted], to [redacted] Specialty Pharmacy in [redacted], Boston. Kindred Pharmacy Director ensured the medication arrived and assisted patient’s son with med box preparation as part of discharge planning. Patient was discharged on the date as planned. On 2/23/2016 Patient contacted Kindred Hospital Boston regarding the “unnecessary” prescription order and was given information about the discharge order process which was initiated and executed by [redacted] Transplant Team. Due to the specialty care of transplant patients the referring hospital manages discharge medication management.
