Initial Business Response / [redacted] (1000, 5, 2015/08/05) */ I am responding to a Revdex.com letter, Please allow me to answer her statement in steps First she states procedures where not done,but billed forIn the case of her husband's deep cleaning (AKA subgingival root planing or SRPS for short) he was seen on 05-21-for a cleaning, xrays and examIt was advised that he have a deep cleaning (SRPS) doneWe submitted to the insurance company for these services and received a pre authorizationHe was then contacted and he scheduled to have treatment done on 06-19-He signed a contract to pay for services, he also signed a practice policy, which states " we make estimates of your dental benefits"I have enclosed copies of the papers for you to review Secondly she states that we do not give an option for filling materialTrue at that time we did not offer amalgam (silver) fillsHowever she was aware, it clearly states in the practice policy that we only use composite or white filling material and that the patient is responsible for the differenceShe signed the practice policy and we point this out to all of our patientsAgain an estimate was given for treatment and it was signed by the patientThe fillings where paid for over a month period with no interest ackled.I have enclosed copies of the papers for you to reviewThird, she states she was told by the office manager that she has a zero balance; that is incorrect, we would not tell a patient who has a balance that they don'tI have enclosed a copy of her bill for you to reviewFourth, she states that we continued to bill for treatment after they stopped coming hereWe submitted to the insurance company for preauthorization for orthodontia for her daughter_ When we received pre authorization we contacted the patient and she choose to scheduleWith most insurances they do not pay all at once the pay quarterly or annuallyThis insurance pays automatic annuallyWe did not bill for services that were not done, it is simply how the insurance company paysI have enclosed the financial contract that the patient signed showing what was expected from the insurance companyIt was our sincerest desire to handle this between the patient and ourselves, however when I tried calling the patient and explain to her that the balance was due, she hung up on meI called back and left message on her voicemailAfter several failed attempts to call the patient and discuss this, she choose to contact the Revdex.com Thank You, [redacted] Initial Consumer Rebuttal / [redacted] (3000, 7, 2015/08/06) */ (The consumer indicated he/she DID NOT accept the response from the business.) I was specifically told by her that my balance was $0 then a year later I received a bill in the amount of $60.00. but again I was told by [redacted] back when I was going to make my last $60.00 payment that the balance was $0.
