From:? ***Date: Thu, Oct 20, at 11:AMSubject: Complaint ID: ***To: ***I wanted to have a chance to response to the letter received by the orthodonticI am requesting a full refund her teeth were not corrected she continues to have an over bite and two teeth are visible on the images that were not corrected? We were told after her braces were removed that the retainer was to be worn daily I called several times and they told me she get use to it they will fill discomfort for the first couple months? On 8/I called the office and stated that I felt her retainer doesn't seem to seat correctly in her mouth? I than schedule an appt but was told by the receptionist that *** *** insurance was not excepted at that office cause it was an HMO state insurance(Our current dental insurance changed) I was told I would have to pay for a new retainer cause the receptionist assume she wasn't wearing her retainer I assure her that after paying thousand of dollars and spending several months being treated I assured her that she was wearing them but she kept assuming., it upset me and I never received an return call for DrKattle after requesting a call back cause I was upset and disagreed with making any more payments? I did not keep the appointment schedule on 8/cause I never received a follow up call and was unable to afford to pay? I went to a new orthodontic that accepted our new insurance and has started treatment and her braces are being applied? As per the requirements of orthodontic treatment your teeth must score a in-order to qualify her over bite therefore her teeth scored high enough to start treatment of new braces? I will be willing to share all medical documentation necessary from her new orthodontics to proved proof that her treatment and over bite was treated incorrectly there for she has restarted the braces treatmentDrKlatte office treated *** but the treatment/services was not completed to satisfactory per the office contract therefor I am requesting a refund of my payments? I understand the insurance company paid I feel that the insurance company paid more than enough an I should be given a full refund due to the incomplete work and the time wasted in addition DrKattle should be aware of his staff and what is communication to the patient/parent? I hold the office staff and DrKlatte accountable for all treatment and stress that I be refunded and move forward with *** new treatmentThank You *** ***? *** ***

From: [redacted]Date: Thu, Oct 20, 2016 at 11:54 AMSubject: Complaint ID: [redacted]To: [redacted]I wanted to have a chance to response to the letter received by the orthodontic. I am requesting a full refund her teeth were not corrected she continues to have an over bite and two teeth are visible on the images that were not corrected.  We were told after her braces were removed that the retainer was to be worn daily I called several times and they told me she get use to it they will fill discomfort for the first couple months.  On 8/4 I called the office and stated that I felt her retainer doesn't seem to seat correctly in her mouth.  I than schedule an appt but was told by the receptionist that [redacted] insurance was not excepted at that office cause it was an HMO state insurance. (Our current dental insurance changed) I was told I would have to pay for a new retainer cause the receptionist assume she wasn't wearing her retainer I assure her that after paying thousand of dollars and spending several months being treated I assured her that she was wearing them but she kept assuming., it upset me and I never received an return call for Dr. Kattle after requesting a call back cause I was upset and disagreed with making any more payments.  I did not keep the appointment schedule on 8/16 cause I never received a follow up call and was unable to afford to pay.  I went to a new orthodontic that accepted our new insurance and has started treatment and her braces are being applied.  As per the requirements of orthodontic treatment your teeth must score a 25 in-order to qualify her over bite therefore her teeth scored high enough to start treatment of new braces.   I will be willing to share all medical documentation necessary from her new orthodontics to proved proof that her treatment and over bite was treated incorrectly there for she has restarted the braces treatment. Dr. Klatte office treated [redacted] but the treatment/services was not completed to satisfactory per the office contract therefor I am requesting a refund of my payments.  I understand the insurance company paid I feel that the insurance company paid more than enough an I should be given a full refund due to the incomplete work and the time wasted in addition Dr. Kattle should be aware of his staff and what is communication to the patient/parent.  I hold the office staff and Dr. Klatte accountable for all treatment and stress that I be refunded and move forward with [redacted] new treatment. Thank You [redacted] [redacted]

12 months after delivery are part of orthodontic treatment and are included in you contracted fee.I assure you it is our goal to achieve the best results for all of our patients, including [redacted], Please let us review some of the facts of [redacted]' orthodontic treatment. Please know that the following is a summary only and that not all appointments are noted. There were a total of 36 appointments scheduled, of which 5 were no shows (patient did not call or show), 5 were rescheduled, 1 cancelled and 29 completed. There were 4 appointments in which loose orthodontic appliances had to be rebonded,-Initial Exam Date: 1/18/2014Contracted and scanned for Invisalign: 2/12/2014-First Invisalign trays delivered: 3/08/2014-Attachments placed:4/09/2014 -It was noted at appointment dated 8/9/2014 that [redacted]' trays were not fitting properly on the upper right side. The poor fit of trays typically occurs from the patient not wearing aligners enough. [redacted] had developed some cavities and scheduled an appointment with her general dentist for restorations. When complete she was to be rescanned for a completely new set of Invisalign trays.-Scanned for new trays; 9/23/2014-New Invisalign trays delivered: 10/22/2014 -Next visit: 12/13/2014 Trays were again not fitting properly, We discussed the [redacted] and her mother the need for cooperation to ensure success.
