They are not addressing my concernsAccording to them, they have been notified and acknowledged that I have had a change of address and phone number, so I am not sure what the problem is there. I made a payment on Nov5th and it was sent back to me Jan11th 2016, since then my account has gone to collections, for what reason I am unsure because it is not due to non-payment, so why has it gone to collections? I have made several attempts to reach someone in the business office, leaving several messages a weekAll of those messages were left with my phone number and still to this day have never received a call backI called and left messages wanting to know why my account went to collections and how I could go about paying this off; I have no idea where to send the funds since my payments have been rejected. I received the income based repayment form from KCH AGAIN in February, so I assumed they needed my income information AGAIN; income which is significantly lower than it was in I did not call in, in March but the hospital did sign for my income based repayment documents on March 17th, 2016; which had my phone number on the paperwork, and still I have not received a phone call on how to go about dealing with repayment

This patient received services in September of 2013. Since that time she has changed addresses and phone numbers multiple timesBased on the information available to us, this patient does not qualify for financial assistance due to her income levels. The last payment on the account was
made in October 2015. A financial counselor spoke with her on March 28, and obtained current address and phone number. Mail has since been returned for wrong address, and when the telephone number is called, the outgoing message says that the Verizon customer is not available and no voice mail has been set up
