Looking through this membership and the concerns; it appears the fitness club has done everything the member has originally requestedThe member originally enrolled on 8/23/2014; on the first page first paragraph it states the day termination notice and the written notice policy (in italic)
(member initialed next to this), 2nd paragraph it repeats the termination policy (bolded) (member initialed next to this as well), on the back of the form, the 10th bullet on the form repeats once again the termination policy (member initial next to this), and finally on this form there is a bolded and boxed out, to stand out portion on the form stating the yearly maintenance fee will be charged once per year; this applies if the membership is active, including a freeze status or within the final days of termination (the member initialed next to this) October 16th, was her yearly maintenance fee.On March 30th, the member submitted a freeze membership request; stating the membership will be on freeze status from 3-30-until 10-01-2015; freeze status is ($per month) Stating that beginning 10-01-she requested her membership go live and the regular membership fee would be charged Her regular membership rate of $would apply beginning 10-01-until a written request would be submitted.On October 17th, the member submitted a day notice of written request of termination of membership Per the termination agreement stated on her membership agreement and termination agreement; the member will be billed if any billing falls within the day notice of termination request Any billing from 10-17-through November 15th, Her next billing will be on November 1st, Allowing club access through November 30th, 2015.The stated member had two payments declined on her freeze membership of $in June and July; there is an automatic service fee of $when monthly dues are declined and there are multiple attempts to receive payment; per the 3rd sentence in the first paragraph of the membership agreement.There is a total negative balance of $on this member's account that we are willing to waive
