11/4/2018Ref : John K*** Sleep DoctorGlacier Hwy Suite , Juneau, AK 9980Fraud DepartmentWhen I originally went in to see John K*** we sat at what appeared to be around dining Room table.During our short conversation his main concern was with a computer or scanner in achair under the other side of the table where he kept trying to scan or make a copy ofmy insurance card.He was very preoccupied with this because or times during our interview hecontinually tried to make a copyDuring our conversation before I even had a chance toanswer his questions in regard to a potential sleeping problem he was already talkingover me, stating well yeah, that sounds like sleep apnea.He then proceeded to want me to sign a form stating that if I didn't bring back hislittle box that I was to strap to my chest and plug in my nose that I would owe him$4,500.00.I was concerned about that for the ob
