Thank you for the opportunity to address the concerns outlined Case #XXXXXXXXAccording to the letter, the complaints are in regards to a dental patient feeling like she was being made to sign a
contract
My investigation of the complaints reveal the following:
In summary, there has been a demonstrated history of multiple no cashows to appointments along with cancellations on the day of the appointment in the time period of XXXX-XXXXIn March of 2017, the dental office was contacted by the patient at which time she inquired as to her balance on her account as well as requested an appointmentThe patient was informed that the balance was zero and an appointment was setThe records show that she did not attend nor call to cancel this appointmentDuring the following five months there were additional appointments made that resulted in three no cashowsOne appointment was excusedA social worker attempted to meet with the patient to determine the obstacles, if any, preventing the patient from making her appointments and assist the patient, but the patient was a no cashow for that appointment as well
In October of 2017, the patient expressed a desire to continue her services at the dental facilityAt this point she was asked to sign a contract of understanding to clarify the expectations and responsibilities for dental patientsIt appears that after this complaint was filed with the Revdex.com three days later the patient did come in to the dental office and voluntarily sign the contract of understanding
Again, thank you for this opportunity to address the concernsBased on the review of the information it appears La Porte Physician Network has been accommodating and continues to work with the patientIt should be noted that at no time was the patient charged for her missed appointmentsWhile La Porte Physician Network would like to provide copies of records as further proof, it cannot do so without patient permission

If I didn't aogn the contract I could never reciwve services there again in and I did not even recieve service from them as proof via my dental recordsSo that's off I have the proof of thatAshley made me leave said I had no appointment when I did for one appointmentThe second I had a car accident and the third was also do to a friends car accident in which time I had seperate surgeriesI am a very sick personI don't drive and I am sedated when having harah surgeries as I did that usually require blood transfusionsI could go into gory details if so wishIf I didn't sign the contract I was never allowed service againThat is forcePeriodIf records need be shown the work on my teeth not done by them for the dates they are mingling on about just as the dentist for the fileMy family is good friends all through life with the graces this has nothing to do with them it has all to do with tbeor secretary who does what she wants and the threat I can't recieve care if I don't sign that is being made to sign is it not?
