you will please review the contract Ms*** signed you will see the amount she paid in full is very different than statedThe total treatment fee for *** was $Half of this was paid by her insurance company, leaving a patient balance of $This amount was divided between herself and her ex husbandThe total amount Ms*** paid out of pocket for ***' treatment was $Out of courtesy we are completing active treatment for her son,Andrew ***Ms***'s personal responsibility for his treatment was $Her out of pocket expense for both children is $3488.75.Ms***'s signed appointment policy and broken appliance policies are also attached for your referenceIt states that emergency appointments are not scheduled in the early morning or late afternoonEmergencies are unfortunately not usually convenient for anyone and are scheduled to make sure the patient is out of painThere is no agreement that repairs will take place at an emergency visitWhile we tried to be as accommodating as possible, Ms*** is very insistent on having appointments when she wants then, whether an appointment is available or notSo much so that she threatened to write a negative review on *** if she was not accommodated And even though she was accommodated she still moved forward with her detrimental review of our practice.It is clear that Ms*** is dissatisfied with the service we have provided, which is why our practice decided it would be in everyone's best interest to end the relationship in hope that she will find another office that she is satisfied with*** has been dismissed from the practice within the legal responsibilities of our practiceWe will not be seeing him for future visits and have offered to send any records needed elsewhere

It has been brought to our attention that the Revdex.com’s main concern of this complaint is whether treatment was paid for but not received.1. For the patient, *** ***: *** was treated for Phase One orthodontic treatmentThis treatment was completed in its entirety on March 25, To be more clear; phase orthodontic treatment is the first of two possible phases of treatment, and it occurs when a young patient is in need of intervention before starting regular orthodontic treatmentThis can result in two different ‘phases’ or treatment plans; each treatment accruing in its own separate feeTherefore, an additional fee will accrue should a second phase of treatment be necessary, with our practice or any other orthodontic practiceBecause all patients do not need a second phase of treatment, it is considered separate and is contracted separately from Phase I. There is no obligation for the patient to stay with the same orthodontist for the second phase nor is the orthodontist obligated to treat a patient once Phase I treatment is completedIt is not uncommon for patients to switch orthodontists for the second phase of treatment. 2. For the patient, *** ***: *** received treatment for Full Phase orthodontic treatment*** has also completed his treatment in its entiretyBraces were removed on July 26, He is now in retainers, thus, our contractual obligations have been fulfilled. We are happy to provide you with whatever information you request if additional proof is needed to show there is no outstanding treatment and that both treatments are complete. In addition, Ms*** was not charged for any office visits for *** following the completion of his Phase One treatmentWe also offered to send the records for *** and *** to whomever they request at no charge to her. I am rejecting this response because: There are patient abandonment and dental safety issues.  One for [redacted] and one for [redacted].#1  [redacted], dental safety issues, patient abandonment?  We asked for an Emergency appnt about 4/5/2017, to fix broken dental brackets causing oral pain, and difficulty eating.  My son was going 700miles out of town for a performance 4/8/2017. The office provided us an appointment but when we got there they said the Orthodontist was not available to fix the problem.  The problem remained, they gave a 2nd appnt, then cancelled it again.  They then pushed out the next appnt for 10 days.  I asked them why they would not fix the issue earlier then 10 more days,  between the hours they are open 9am to 2pm., they said they are booked out 8 weeks, they did not have opening for Emergency visits, and if I wanted I could wait until my appnt in July 2017.   Failing to perform emergency services timely impedes care and reaching treatment goals, it also left my son with oral pain and difficulty eating.  The dental  emergency visit was not provided by the office for almost 19 days, on 4/27/2017.     You can see this issue documented on www.yelp.com Laguna Orthodontics, Elk Grove, CA back 4/2017.   #2  [redacted], agreement to provide services on 7/26/2017, and refusal to provide those services, with possible /patient abandonmentOn the 4/27/2017 visit the office staff gave me 3 follow up visit.  Two visits for [redacted], and one visit for [redacted].    I kept the appointment slips.  [redacted] remains under treatment with Dr. [redacted] he is in the middle of a staged treatment plan.     We  had an appointment for [redacted] for a  follow up exam, retainer check, and were going to discuss stage II of his Orthodontic treatment.    Dr. [redacted] sent me a letter stating he was terminating [redacted] care ( in the middle of a staged treatment plan).  I called the office to confirm the appointment for the exam, retainer check, and discuss what goals were completed or not completed, and discuss stopping staged care.   The office staff just told me, "no we cancelled your appointment."    I told the office receptionist that  I did not agree to cancel the appointment, and that I need to know about my son's treatment status.   The office Receptionist  said to have another Orthodontist call them.  Basically the office is refusing to discuss treatment with the childs authorized representative.  They refuse to provide information on the childs current condition, and unilaterally cancelled an appointment I had agreed upon for services that are already paid for.  The services are usual and necessary, especially if the Orthodontist wants to terminate  staged care.  Unilaterally cancelling a set appnt for 7/26/2017 seems like patient abandonment and withholding of promised and paid for services.  In addition to that requiring me to HIRE another Orthodontist to find out about my son's current dental condition seems inappropriate and possible patient abandonment.  Are they going to reimburse me for care so I can hire an Orthodontist to be advised of my son's current condition? # 3  The contract the provider sent you is not the combined total cost to me for both boys services, nor does it include what my health plan paid them for their services.  This provider got about $8K for services they are failing to complete.   If the provider is refusing to complete final services for [redacted] or [redacted] then they should refund me for the services, as I will have to hire another Orthodontist (per their receptionist)  to complete the services they are refusing to complete? I can't even get a final assessment on [redacted].    Please let the provider know I will show up with [redacted] to his appointment at 7/26 at 8:30 am, as agreed, for oral exam, retainer check, and to discuss his treatment plan and goals met and not met for this staged treatment plan.  I expect him to keep the appointment as agreed.   Please let me know if he is refusing this appointment and abandoning patient care.  Kind regards,  [redacted], Mother for [redacted] and [redacted]
