I am attaching a a financial arrangement worksheet that was sent to me by Laidlaw Orthedontics to inform me of my responsibility, when it came to my sons ***'s bracesThis financial arrangement was dated on January, 19, 2016, it was created without my knowldege, nor my consentIt was created solely on the statement from my ex husband that I would be paying percent of the billLaidlaw has stated to you and I, that they were never holding me financially responsible for for anythingThis form that was sent to me clearly shows that to be untrueWhen I spoke to them on the phone, they said they knew from my ex husband that I was financially responsible for percentThis situation is clearly unethical on Laidlaws part, so they could close the sale, they chose not to get my consent for braces for my son or for the financial arrangementLaid law did remove me from the arrangement when pressed to do so, but I do not believe the vast majority of consumers out their would feel comfortable to do soIn issues of joint custody Laidlaw should have all financially responsible parties consent before proceeding with serviceI believe this shows Laidlaw Orthodontics to have questionable ethical standards, its not right that people should be put into this situation, I think people should be aware this can happen

In regards to the complaint made by Ms. [redacted], I believe that the situation is a miscommunication betweenher and her ex-husband. I called and assured her that we do not have a signed contract with her name on it, nordoes she owe us any money now or in the future. Her ex-husband sought...

treatment and agreed to pay for treatmentin full for his son as reflected by the contract for treatment signed by Mr. [redacted] on March 14, 2016. I calledher personally to explain the situation and let her know that we would not bill someone purposely who does nothave an active account with our office which she does not. The only paperwork we have with [redacted]'s name on it isa "Financial Arrangement Worksheet" that Dad used to estimate proposed payments at the initial exam in January,but when it came time to actually consent to treatment and sign a contract, the patient's father, [redacted],was the only one to sign a contract (enclosed) and is therefore responsible for the account with our office. I alsocalled and spoke with Mr. [redacted] who gave me oral consent to share his contract with you and withMs. [redacted]. He also stated that he had already sent Ms. [redacted] a copy of the contract as well. He understands thathis contract is with us, and we do not have anything to do with any separate financial arrangements he may hawwith his ex-wife.I further assured Ms. [redacted] that if she ever had any questions or concerns in the future, that she should always feelfree to call me first rather that feeling she had to go to the Revdex.com. We pride ourselves on excellent patientrelationships and always work to handle any confusion when it arises in situations like these. Please feel free to callif you have any other questions about this issue, but the bill in question to Ms. [redacted] does not exist.

This letter is in response to claim ID #***.  I am writing to confirm the following information:
 
Laidlaw Orthodontics does not have a contract with *** ***.
*** *** has never been sent a contract or bill for any money due from her to our office.
***’s ex-husband Mr. *** signed a contract for his son’s treatment fee in full.
Mr. *** emailed his ex-wife copies of his contract per his divorce decree, but we are not involved in that matter.
*** *** received one appointment reminder email from my office at the beginning of her son’s treatment as per her ex-husbands request, but she called the office to request she not receive those and her email was removed from her son’s chart.
Dr. L*** called *** once notification of this complaint was received to clear up any confusion that was present on ***’s part and confirmed again that *** did not owe us any money nor have a contract or balance with our office.
