This is in response to your letter advising a complaint was filed by the above named individual
I apologize for the delay in responding as this letter was sent through the wrong internal channelsI reviewed the information supplied by Mr*** as well as our billing systemThe
services provided to Mr*** on 1/27/were billed under DrV*** who was a new physician under the group practice of Lake County EKG and was not yet contracted with SummacareAs a result, when the claim was processed by Summacare, it left a patient balance of $A statement was mailed on 4/27/and 5/25/to the patient for this amountThe patient did call on 5/5/requesting that the matter be looked intoThe patient called again on 6/17/advising he received another statement and what the status of the matter wasThe patient did send his in-network balance due of $which was posted to his account on 7/23/Although his account was in review with our Provider Relations department to resolve the contract issue with Summacare, his balance was never suspended during that process which would have caused statements to stop generatingThis was clearly an error on our part
As of August 24, the matter was resolved and Mr***'s account was cleared and reduced to a zero balance (see attached)Also, see the attached letter that was sent to Mr*** on this matter
