[To assist us in bringing this matter to a close, you must give us a reason why you are rejecting the responseIf no reason is received your complaint will be closed Administratively Resolved] Complaint: [redacted] I am rejecting this response because: This response is being rejected because DrI***'s office would not release my x-rays from 1/31/and because I had to see another dentist Dr [redacted] on 3/7/2015, for the same procedureI have received ther chedk from [redacted] in the amount of $220, it posted to my account on 3/2/From 3/2/- 3/6/2015, I was in New Orleans on government travel, when I began experiencing a toothacheI contacted DrI***'s office and scheduled an appointment for 3/7/The entire time that he was performing whatever procedure, I cried, and he stopped several times to ask me if I was okAfter the procedure, he prescribed Rx's that I had filled at ***When I returned home I was in constant pain, my husband suggested that I see another dentist if the problem persistDue to the pain, I contacted Dr [redacted] 's office and scheduled an appointment for 3/9/Dr [redacted] then performed the same procedure that I supposedly had done on 3/7/I will not pay for the x-rays that DrI***'s office would not send to Dr [redacted] after several telephone requests, or the whatever procedure that he performed on 3/7/2015, because I have already paid another dentist for these services Regards, [redacted] ***

June 1, 2015In response to the Revdex.com complaint for a billing and collection issue on [redacted].[redacted] originally contacted us on January 20, 2015 and asked if we were providers for her insurance Company, [redacted]; we advised we are In network provider and she proceeded to scheduled a new patient...

appointment. [redacted] requested a Saturday appointment and was scheduled for Saturday, Jan 31, 2015 for a new patient appointment. At this appointment, [redacted] had a complete comprehensive evaluation exam - and a full mouth Series of x-rays , a prophy cleaning,and a treatment plan was discussed for the fixture treatment that was needed. She was advised that she needed to have a periodontal scaling and root planning on all 4 quadrants (UR, LR, UL, LL) of her mouth and possibly a periodontal surgical procedure, Upon completion of her appointment the estimated cost for her treatment was discussed and acknowledgment was signed (attached). [redacted] accepted treatment and scheduled her next visit for the next Saturday we were available, March 7, 2015We billed out the insurance company for exam $100.00Full Mouth Series of Xrays 120,00Prophylaxis - Adult 100.00The claim was sent on 1/31/2015, in the amount of $320,00, Attached you will see the Explanation of Benefits from the insurance company which processed the claim on 2/17/2015. The insurance mailed the claim payment to the patient, and according to [redacted], the check was cashed on 3/2/2015.We made the appropriate write-offs that is required due to being a in-network provider which left the patient with a balance of $220, in which, if the insurance company would have sent us the check there would not have been a payment to collect from the patient. WE did not charge her any payment in January 31, 2015 as we did not anticipate the insurance sending her the check instead of the officeOn Saturday, March 7, 2015, she was seen for a periodontal scaling and root planning on the Upper Left side and the Lower left side of her mouth. She was advised that she had an infection on tooth #8, which is on the upper right side of her mouth. Dr I[redacted] explained that it may need a root Canal and it would be reevaluated at her next appointment, which was for a periodontal scaling and root planning on the Upper right side and lower right side. She was prescribed an antibiotic to take for the infection.On Monday, March 9, 2015, Dr I[redacted]'s assistant called the patient regarding her tooth & left a message to call us back. Several messages were left between March 9 and March 12 but she never returned our phone calls.On Saturday, April 11, 2015, [redacted] had a scheduled appointment to have a periodontal scaling in the upper right side and the lower right side of her mouth. We called 2 days in advance to confirm the appointment for 4/11/15, and left message to have her call us to confirm that she will be at her appointment on Saturday, Patient did not call nor show up for her appointment.She stated that Dr [redacted]'s office called to request her x-rays on several different occasions and hid no success. Dr [redacted]'s office did call and request x-rays for the patient but, by law, the patient needed to sign an authorization to release the x-rays to be sent to Dr [redacted]'s office. This was explained to Dr [redacted]'s office that we needed to first have the release form signed and then we could send them Again, we attempted several phone calls to [redacted] and left messages stating we needed to have her sign a release form so we could send the x-rays, but she did not return any of the phone calls. We still do not have a consent form signed to release the x-rays so they have not been sent to Dr. [redacted]'s Office,[redacted] received a statement in the amount of $572.00 for services that were truly rendered. AGAIN, her insurance company mailed the claim payments to the patient and the patient cashed the check on March 2, 2015. (see attached copy of insurance explanation of benefits) Our office has made the necessary adjustments for the services rendered due to being a in-network provider and [redacted] was billed for the balance. She has not paid us any money for the services that were rendered (see attach copy of patient ledger), therefore, I cannot justify the $820 that [redacted] is disputing when she has not, nor the insurance company, paid us $820.If she does not make payment on her account, appropriate action will be taken,
