We received your complaint from the Dale ***, a patient of LCHC Below is our written verification of the situation that you requested. Lakeshore Community Health Care is a non-profit organization that provides comprehensive medical, dental and behavioral health services to residents
of *** and *** Counties. Our mission is access to care without barriersWe provide discounted fees to those that are 200% below the Federal Poverty Level(FPL) We have a sliding fee schedule that is based on patient’s incomePatients are required to provide proof of income to qualify for these discounted feesIf patient’s income falls below 200% of the FPL, they then qualify for a co-payment that ranges between $and $As we receive Federal funds, we are not allowed to give discounted fees to anyone whose income is 200% above the FPL. The complainant was see in our facility on 5/31/As an uninsured patient requesting discounted fees, he was asked for proof of income to determine if he would qualify. He provided his SSI statement as proof of income, stating the SSA payments would start in June of The patient was told that he would also need to provide his wife’s proof of income within the next two weeks, to accurately determine if he qualified for discounted feesHe was asked to make a $payment towards his charges for this date of service. The complainant brought in his wife’s proof of income on 6/23/At this time, it was determined that with their combined income, they were above 200% of the FPL and did not qualify for discounted fees. On 8/23/the complainant contacted the billing department of LCHC disputing his bill. This complaint was brought to my attention. After review, I determined that as his SSI did not start until June of 2017, his co-payment for his 5/office visit should be adjusted to a $co-payment His account was credited $The required paperwork was process to refund the $to the patient The patient was notified of this creditHe was also informed that based on their income, they do not qualify for discounted fees for any future services
