I am sending this in regards to a complaint made against our company, LakesideEndodontics. We have spent the past ten years building a practice based on trust &honesty.We saw the named individual for endodontic treatment 01/24/2014. Prior toseeing our patients we collect insurance information...

and verify with the given insurancecarrier. Once this information is obtained we calculate the patients estimate out of pocketexpenses for the treatment and relay this information to the patient. Upon arriving at ouroffice the patient received several forms that clearly stated our office financial policies aswell as how we bill to dental insurance carriers. As you can see from the attacheddocuments the named patient signed a financial agreement indicating that we have noway of knowing or receive no guarantee from the insurance carrier of payment until theclaim is received. We informed the patient of the estimated out of pocket expenses priorto the schedule appointment and collected that portion upon completion of the services.As to the issue of being in the preferred network of [redacted] this information was nevergiven to the patient. We have never been preferred providers with [redacted]. We can and dobill to them regularly but have never been preferred. The patient stated to me that we toldthem that we were in fact preferred providers with the insurance carrier. I apologized tothe patient for any miscommunication but stated to him that we neither are nor have everbeen so. The patient went on to say that since the general dentist referred them to us thatwe were preferred and that they (the general dentist) would not send them to someone outof their network. I informed the patient that his dentist does not refer based on insurancenetworks but on who they work with & who does exceptional work. I tried to explainthis to the patient and come to an understanding but was told that is we make them paythe balance that they would in turn report us to Revdex.com for this. I had hoped that this wouldnot be the case but there was nothing more that I could do for the patient. I explainedeverything to them multiple times but was met with no resolution.Please review this information and if there is anything further needed contact ouroffice. I have enclosed a copy of the signed financial agreement from the patient.Thank you for your time,
