We received notification of complaint ID [redacted] on 10/20/and we will do our very best to come to a positive conclusion in this caseWe received the [redacted] release form from the patient via the Revdex.com on 11/10/and have done our very best to respond in a timely manner We are very sorry that the patient was dissatisfied with her treatmentHer frustration is understandable since this has been ongoing since June of when she decided to start her restorationsHer original diagnosis was in October of We tried to communicate with her in order to solve the issues that she was having, however, she would not return any of our calls or come in so that we could address her concernsIt is truly impossible to do anything without communication and we were more than willing to work with her as well as for herThis was never about the money for us, as we told the patient that we were content to wait on payment, per her request, until we had permanently cemented her bridgeWe were waiting on permanent cementation do to the fact that one of the teeth supporting the bridge had deep decay and was symptomatic giving us the indication that it may require a root canalIn regards to the “2nd bridge that broke into pieces,” it was made of temporary material by another dentist and was deterioratingWe made a temporary repair to carry her through until she was ready to start the next phase of her treatment We were unable to finish treatment do to the fact that she would not return our callsShe went elsewhere without giving us the opportunity to complete treatment and didn't notify us that it had been done until we received this complaint from the Revdex.com which was placed after she received notification that her account had been sent to collectionsWe sent the account to collections only after numerous attempts to contact the patient went unanswered We are also very sorry that she incurred an unexpected charge due to the fact that her insurance did not pay for her recall visitWe base our information off of what was provided to us by the patientShe had been to another dentist in between visits with our office, therefore, her insurance would not cover the expenses at her recall visit We will dismiss the remaining balance on her account, as she completed treatment elsewhere according to the information we received from the Revdex.comWe wish her well with her new dental provider Regards, [redacted]

For your reference, details of the offer I reviewed appear below.This response from the dental office is not acceptable and not entirely trueMy work on this bridge in question was completed in June of I was asked to wait for "some time" to see how the bite was before it would be set as permanentI asked for a specific time, I was given "whenever" as a time to go back to "fix" all of the issues that were stated from day Beginning from the day I left the office, I called them within an hour or two concerning all of the issues that were present from his work on this bridgeI had to call them because I was not given a mirror to see anything before I left the officeOnce I arrived at home, I noticed damage on a good tooth of mineThis tooth is the one in front of the bridge and now had a big scratch, where a chunk of it was missingWhen I told them this, it was originally denied that it happened in the office but hesitantly repaired it a few days laterJust to note, if I had a chunk missing from a tooth prior to this replacement bridge, the hygienist or dentist would be the first to tell me to get that repairedAs I had a cleaning visits there every monthsWith that resolved, I still had a list of issues with the new bridgeMy issues were that it did NOT look natural, when I smile the dark metal is visible This dentist replaced a bridge that I had from the 80sMy old bridge was very natural in looks and biteUnless I told people, they would not know it was a bridgeThat was done almost years ago! I would think that with technology during these last years would have improved the process and aestheticsI also stated to the dentist that there was a very sharp metal sticking out inside of the bride and it hurt my tongue every time it would touch itNothing was done about that or the looks I waited from June until October to go back as it was my cleaning appointmentBut during those months I had called and emailed with my concerns listedIn October 2013, after my cleaning, the dentist walked in to check on the cleaning and also make this bridge permanent as I had NOT been able to use it at all since June Dentist told me that he prefers to leave it temporary because if anything goes "wrong" it would be easier for him to fix itSo therefore did not make it permanentI asked at this time what real issue was in not making it permanent and got a response of his worry of maybe the one of the tooth may not support the bridgeOriginally, in all x-rays, it showed a small decaySo, one of my first thoughts was that if you (dentist) began to work and saw more decay than originally stated, I should have been notified or better yet SHOWN while I'm sitting there in the chair in front of you(dentist) doing the work with a mirrorHe also mentioned that more of the tooth was taken out! It's difficult as a patient to sit there and believe anything when work is being done and not explainedIf the x-ray shows small decay and the patient is told one thing - but if as you begin to get working and realize that there's more decay that was not visible - you (dentist) should bring a mirror and discuss and show the patientNot very unreasonableMy dentists in the past revealed everything prior to any work and showed me every step to share information with meNot a difficult processThis is information I should have been told in June Instead of letting me to go for months without being able to use that side of my mouth for MONTHSOverall, I gave them plenty of time and opportunity to come up with ideas or to even let me know of HOW I can actually EAT with this new bridge! I have emailed the office of all of these and more concerns and also emailed about how this temporary bridge popped out of its place while I was at a restaurant eating mashed potatoes (softest food possible)How embarrassing, but also dangerous if I had swallowed this?! Bad enough I chewed on my own "teeth"In all, the work was completed in June of and I waited until March-April of to look for 2nd opinionsI believe allowing them 9-months to resolve these issues was plenty of time As for the billing, we had discussed that the price would be split into equal paymentsI paid the first payment upon when the impressions were taken, the 2nd payment was paid after the work in June was completed and final payment would have been when it was finalized, placed as permanent which never happened To reply to the comments made from the dental office, the issue with my complaint is ALL about the money as I have been billed SEVERAL times a WEEK since June of I even have a copy of my email as I complained to them about receiving bills several times a week! I am sure I can present several dozen bills and threats of collection agencies As to a few other details that were stated in their response (which is at the bottom of this reply), I will reply to each of the said comments made by the officeIt was stated that I did not go in to get things repaired and that they were willing to work with me in satisfying my concernsThis is not true because I was sitting in the chair at the office and telling the dentist face to face each of the issues and all I received were excusesFor example, I told him I did not like the metal showing when I smileHe said that the metal hold is stronger and it is neededI told him that my old bridge that he took out had metal in it as well and that lasted almost years but yet had a natural feel and look to itHe had nothing further to saySo how would me returning to the office months later help? Would the dentist remake the bridge to cover up the metal that is visible? The answer is no! Another example, I told the dentist (face-to-face) about the SHARP metal that catches my tongue from the inside of the bridgeI was right there in his chairDid he take it out and try to fix that? NO! So the reason I did not go back was because when I was right there in person telling him face to face of each of my concerns, nothing was doneI did give it time and plenty of it to come up with ideas to resolve this and it was apparent they just wanted to be paid! As for me going to another dentist, again I do have an a copy of my email that I sent them (this dental office) saying that I am going to go to a few dentists as a second opinion, which was March - April of 2014! I never visited any other dentists as their office is claims That is a blatant lie and my insurance office can CLARIFY that instantly! As every dentist office does, any patient walking in the door, the first two things they REQUIRE are your license and insurance cards and make copies of themThe dental office is VERY WELL informed by what is and is not covered! I can get statements from my insurance to verify that NO other dentist was ever involved. Lastly, they have stated in their response that they "will dismiss the remaining balance on my account"Although that makes them look good, it's all just words to me until I see them retract it from the credit bureaus in which they have reported to under my name and again my husband's nameThey have ruined my credit score and my husbands credit scoreIt will remain there for years as a negative markI would like to have proof of this dismissal in the form of a letter in order to have it deleted from the credit bureaus' recordsSince they involved the collection agency to report this, they need to contact the agency to clear this as a debt. We tried to communicate with her in order to solve the issues that she was having, however, she would not return any of our calls or come in so that we could address her concerns. It is truly impossible to do anything without communication and we were more than willing to work with her as well as for her. Revdex.com spoke with [redacted] from the company and she stated that in order for the company to respond, they would need a signed [redacted] form from the consumer.

Review: The initial work the dentist was to complete was to replace two bridges and add an implant. Beginning with the first replacement of the 1st bridge, while taking impressions of all teeth - my 2nd bridge broke into pieces and got stuck in the impressions he took. He allowed me to leave his office with a hole in my bridge. I had to call to return so that he can fill in the hole that was left open for over a week and I could not eat on that side. He filled that in while beginning to replace my other bridge. Not sure as to why we wouldn't have started with the bridge that just fell apart in his office! Anyways, he replaced the 1st bridge in June 2013 and left it with a temporary glue so that he can adjust if needed. I could not chew on this new bridge for months and I called several times and shared each of my concerns with them. I was told to give it more time and wait for my October cleaning appointment and he'll look at it then and make it permanent. Went in October for my routine cleaning - which had been free of charge due to my insurance paying for it. This was not free! Should have been notified PRIOR to my cleaning but was not. I was not aware they were going to charge me for this or I would have preferred to wait 2 months and it would have been free. At this time, they also wanted to do a full mouth xray which I declined, only because we had a plan of all the work needed to complete. Glad I turned down the xray because that would have been several hundred more that I wouldn't have expected to pay out of pocket! To make it brief, in October 21013, he did not make my bridge permanent and I had a LOT of trouble with it. Waited until March/April of 2014 to go into a dentist for a 2nd opinion and had it fixed at that time. So Levenson did NOT complete the work on the bridge. I believe giving him over 9months was enough time to repair his own work. So I don't feel I owe him this last payment of the bridge due to him NOT completing it! He did NOT complete his work, shouldn't be paid in full.Desired Settlement: He has already damaged my credit by reporting it to all three credit bureaus and also damaged my husbands credit as well. In which, my husband has had NOTHING to do with any of this. I want the office to clear me of this last payment that they claim I owe and wipe the report with the credit bureaus. The next thing I file will be with the court system within 30-45 days.

We received notification of complaint ID[redacted] on 10/20/2014 and we will do our very best to come to a positive conclusion in this case. We received the [redacted] release form from the patient via the Revdex.com on 11/10/2014 and have done our very best to respond in a timely manner.

We are very sorry that the patient was dissatisfied with her treatment. Her frustration is understandable since this has been ongoing since June of 2013 when she decided to start her restorations. Her original diagnosis was in October of 2012. We tried to communicate with her in order to solve the issues that she was having, however, she would not return any of our calls or come in so that we could address her concerns. It is truly impossible to do anything without communication and we were more than willing to work with her as well as for her. This was never about the money for us, as we told the patient that we were content to wait on payment, per her request, until we had permanently cemented her bridge. We were waiting on permanent cementation do to the fact that one of the teeth supporting the bridge had deep decay and was symptomatic giving us the indication that it may require a root canal. In regards to the “2nd bridge that broke into pieces,” it was made of temporary material by another dentist and was deteriorating. We made a temporary repair to carry her through until she was ready to start the next phase of her treatment.

We were unable to finish treatment do to the fact that she would not return our calls. She went elsewhere without giving us the opportunity to complete treatment and didn't notify us that it had been done until we received this complaint from the Revdex.com which was placed after she received notification that her account had been sent to collections. We sent the account to collections only after numerous attempts to contact the patient went unanswered.

We are also very sorry that she incurred an unexpected charge due to the fact that her insurance did not pay for her recall visit. We base our information off of what was provided to us by the patient. She had been to another dentist in between visits with our office, therefore, her insurance would not cover the expenses at her recall visit.

We will dismiss the remaining balance on her account, as she completed treatment elsewhere according to the information we received from the Revdex.com. We wish her well with her new dental provider.

For your reference, details of the offer I reviewed appear below.This response from the dental office is not acceptable and not entirely true. My work on this bridge in question was completed in June of 2013. I was asked to wait for "some time" to see how the bite was before it would be set as permanent. I asked for a specific time, I was given "whenever" as a time to go back to "fix" all of the issues that were stated from day 1. Beginning from the day I left the office, I called them within an hour or two concerning all of the issues that were present from his work on this bridge. I had to call them because I was not given a mirror to see anything before I left the office. Once I arrived at home, I noticed damage on a good tooth of mine. This tooth is the one in front of the bridge and now had a big scratch, where a chunk of it was missing. When I told them this, it was originally denied that it happened in the office but hesitantly repaired it a few days later. Just to note, if I had a chunk missing from a tooth prior to this replacement bridge, the hygienist or dentist would be the first to tell me to get that repaired. As I had a cleaning visits there every 6 months. With that resolved, I still had a list of issues with the new bridge. My issues were that it did NOT look natural, when I smile the dark metal is visible. This dentist replaced a bridge that I had from the 80s. My old bridge was very natural in looks and bite. Unless I told people, they would not know it was a bridge. That was done almost 30 years ago! I would think that with technology during these last 30 years would have improved the process and aesthetics. I also stated to the dentist that there was a very sharp metal sticking out inside of the bride and it hurt my tongue every time it would touch it. Nothing was done about that or the looks. I waited from June 2013 until October 2013 to go back as it was my cleaning appointment. But during those months I had called and emailed with my concerns listed. In October 2013, after my cleaning, the dentist walked in to check on the cleaning and also make this bridge permanent as I had NOT been able to use it at all since June 2013. Dentist told me that he prefers to leave it temporary because if anything goes "wrong" it would be easier for him to fix it. So therefore did not make it permanent. I asked at this time what real issue was in not making it permanent and got a response of his worry of maybe the one of the tooth may not support the bridge. Originally, in all x-rays, it showed a small decay. So, one of my first thoughts was that if you (dentist) began to work and saw more decay than originally stated, I should have been notified or better yet SHOWN while I'm sitting there in the chair in front of you(dentist) doing the work with a mirror. He also mentioned that more of the tooth was taken out! It's difficult as a patient to sit there and believe anything when work is being done and not explained. If the x-ray shows small decay and the patient is told one thing - but if as you begin to get working and realize that there's more decay that was not visible - you (dentist) should bring a mirror and discuss and show the patient. Not very unreasonable. My dentists in the past revealed everything prior to any work and showed me every step to share information with me. Not a difficult process. This is information I should have been told in June 2013. Instead of letting me to go for months without being able to use that side of my mouth for MONTHS. Overall, I gave them plenty of time and opportunity to come up with ideas or to even let me know of HOW I can actually EAT with this new bridge! I have emailed the office of all of these and more concerns and also emailed about how this temporary bridge popped out of its place while I was at a restaurant eating mashed potatoes (softest food possible). How embarrassing, but also dangerous if I had swallowed this?! Bad enough I chewed on my own "teeth". In all, the work was completed in June of 2013 and I waited until March-April of 2014 to look for 2nd opinions. I believe allowing them 9-10 months to resolve these issues was plenty of time. As for the billing, we had discussed that the price would be split into 3 equal payments. I paid the first payment upon when the impressions were taken, the 2nd payment was paid after the work in June was completed and final payment would have been when it was finalized, placed as permanent which never happened. To reply to the comments made from the dental office, the issue with my complaint is ALL about the money as I have been billed SEVERAL times a WEEK since June of 2013. I even have a copy of my email as I complained to them about receiving bills several times a week! I am sure I can present several dozen bills and threats of collection agencies. As to a few other details that were stated in their response (which is at the bottom of this reply), I will reply to each of the said comments made by the office. It was stated that I did not go in to get things repaired and that they were willing to work with me in satisfying my concerns. This is not true because I was sitting in the chair at the office and telling the dentist face to face each of the issues and all I received were excuses. For example, I told him I did not like the metal showing when I smile. He said that the metal hold is stronger and it is needed. I told him that my old bridge that he took out had metal in it as well and that lasted almost 30 years but yet had a natural feel and look to it. He had nothing further to say. So how would me returning to the office months later help? Would the dentist remake the bridge to cover up the metal that is visible? The answer is no! Another example, I told the dentist (face-to-face) about the SHARP metal that catches my tongue from the inside of the bridge. I was right there in his chair. Did he take it out and try to fix that? NO! So the reason I did not go back was because when I was right there in person telling him face to face of each of my concerns, nothing was done. I did give it time and plenty of it to come up with ideas to resolve this and it was apparent they just wanted to be paid! As for me going to another dentist, again I do have an a copy of my email that I sent them (this dental office) saying that I am going to go to a few dentists as a second opinion, which was March - April of 2014! I never visited any other dentists as their office is claims. That is a blatant lie and my insurance office can CLARIFY that instantly! As every dentist office does, any patient walking in the door, the first two things they REQUIRE are your license and insurance cards and make copies of them. The dental office is VERY WELL informed by what is and is not covered! I can get statements from my insurance to verify that NO other dentist was ever involved. Lastly, they have stated in their response that they "will dismiss the remaining balance on my account". Although that makes them look good, it's all just words to me until I see them retract it from the credit bureaus in which they have reported to under my name and again my husband's name. They have ruined my credit score and my husbands credit score. It will remain there for years as a negative mark. I would like to have proof of this dismissal in the form of a letter in order to have it deleted from the credit bureaus' records. Since they involved the collection agency to report this, they need to contact the agency to clear this as a debt. I was and still am very upset that this bridge was such a huge headache instead of a quick replacement. All in all, I have had a bad experience. Here it is two years later according to their records, and I still need to have both bridges replaced. With two years gone by, I'm still at square one and worse off.
