I have reviewed the response made by the business in reference to complaint ID [redacted] , and have determined that this does not resolve my complaint For your reference, details of the offer I reviewed appear below Regards, [redacted] In Sarayah's response to my complaint"Trainee has three choices regarding where they are comfortable training such as online, private, or at the [redacted] .My coworker and I did book a private session, but it was never mentioned in any form of communication that it would be at her personal residenceHer website [https://sites.google.com/site/lashtowncleveland/home] and also her StyleSeat account [https://www.styleseat.com/lashtowncleveland?q=lashtown+cleveland&pref=search_v2... both state that Eyelash Extension Training will be located at [redacted] *** [redacted] [redacted] ***"Lashtown Cleveland is not nor has never implied that they are a school .When you search 'Lashtown Cleveland' on [redacted] or [redacted] search, it comes up labeled as a Beauty SchoolWhich is very misleadingHer home address is labeled as the 'Beauty School' address in these searches(reference in attachments)"Lashtown Cleveland has never stated that they are registered with the board of cosmetology nor implied such; due to the fact it is not regulated as such and we are using all medical grade equipment .On both Groupon Sarayah labels herself as a Salon (reference in attachments)On StyleSeat, Sarayah claims herself to be an Esthetician(reference in attachments)- which are both licenses given by the Ohio State Board of Cosmetology though she has no record of receiving a legitimate license according to the Ohio License websiteAs I know that Eyelash Extensions are not regulated by the Ohio State Board of Cosmetology, you can not claim yourself to have professional credentials without having those credentials No matter whether 'medical grade equipment' or not; these services need to be practiced in a sanitary environment'Medical grade' or not, there was no place for us to sanitize our toolsSarayah demonstrated bottom lash extensions on both my coworker and I, without sanitizing her tweezers in between "Trainee signed a binding contract stating that they would not do such a thing"I never received an emailed contract nor a copy in person with Sarayah A [redacted] signature on this contractEven in the copy that I was forwarded by the Revdex.com- she still didn't have her signature and I never signed my name eitherAlso in the contract that we were given, her legal name is stated as "Sarayah M***" which causes some confusionIn her contract that she had sent us there were many expectations of what we would be learning in the class that I did not feel we touched base on or learned adequate knowledge onSuch as State Laws and Local Laws/Bonding Requirements; the information that was given is that we are required to purchase a medical bond per individual lash stylist to perform these services- we found out that this was not trueWe were also told that you are only allowed to perform lash extension services in a separate room aside from any other salon servicesThis is also not true as Eyelash Extensions are NOT regulated by the Ohio State Board of Cosmetology.Product Knowledge/Client Preparation; She touched base on a some types of glues and lashes that she prefers, also went over some of the chemicals found in different types of gluesShe never went over products used for cleansing the eyes or prepping the lashesShe also never demonstrated on the mannequin, myself, or my coworker how to properly prepare the eye for a lash serviceTools and Use; In response to "There was an offer submitted and the consumer refused while still being certified. We do not issue refunds once a student is certified" -I am confused about what this is referring toIf it is referring to my request for a refund, then this statement is completely untrueNo, an offer for refund was never submitted during the time of training to the time of becoming certifiedI paid $for a deposit, and before we took our break for lunch Sarayah said that she needed the remaining balanceWhich was supposed to be an additional $She added an additional $fee that she claimed to be for "tax" which she has still in our messages to the Revdex.com has not explained what we were being taxed on- as education can not taxed. Everyone does learn differently- but in this "class" crucial information on safety, sanitation, proper care of the eyes and eyelashes post service was not explained/demonstrated properly and some parts not explained at all. In Sarayah's response "You are bonded through our training program for health and saftey reasons.". Sarayah has a bond posted in her house in the dining room area that we were training inMy coworker and I after some investigation found out that this bond expired in March of That means that myself, my coworker, and any other trainees between March and now- have been falsely told that we were bonded 'for medical and safety reasons' and that us being bonded through Lashtown Cleveland was included in our training. In Sarayah's response "If you choose not to bond your place you are putting yourself and others at risk it is required under the medical board that if you are using all medical grade equipment that you must be bonded or work under a doctor or nurses supervision."The tools that we used to apply lash extensions for this 'training course' were two sets of tweezersAfter being confused about what tools we used that are considered 'medical grade' (or why I would need to be bonded or have a nurse/doctor supervise me in order to use tweezers on a client), my coworker and I did some research with The State of Ohio Medical BoardThis information was revealed to be alsoWhile performing eyelash extensions- we are not cutting into the skin or doing anything relating inside the body so therefor eyelash extensions are no concern of The State of Ohio Medical Board. In the responses that I've received from Sarayah I feel that all of the points I have made explaining how this 'training course' was misleading and untruthful- she has not addressed any of the issues that I have brought upInstead is avoiding every valid point that I am bringing up and turning it to say that I am offered a year of ongoing training, that I am bonded through her business Lashtown Cleveland, and that she never claimed to be a Beauty School/Salon nor a licensed professionalI will attach the images again that show she DOES claim to be all of of these thingsAgain, I was not informed that this class would be inside someone's dining room. I would not have taken this class if I would have known it would be inside of someone's dining room with a child and cat running around, if I knew that it was her home address that was labeled on Google as Lashtown Cleveland- Beauty School, or that I would receive information that I was protected under a bond with a company for training when in reality the bond is expired

In response to Sarayah A*** response on complaint # ***. (Sarah’s comments are in small regular font and mine are in bold italic.)
There was an offer submitted and the consumer refused while still
being certified"We do not issue refunds once a student is
certified." Everyone has a different learning style this is why we give
you a year of support with our packageI would have no problem giving back this certificate and kit received
from this fraudulent company for the full refund. The offer that was extended, was extended before we even
left the class because it is part of her protocol not because we filed a
complaint with the Revdex.comSo she is doing nothing to make this right You are bonded through our training program for health and saftey
reasonsIf you choose not to bond your place you are putting yourself and
others at risk it is required under the medical board that if you are using all
medical grade equipment that you must be bonded or work under a doctor or
nurses supervisionSo about this bonding, which for simple terms means “INSURED”. Now, I have contacted the company that “BONDED” the trainer and her
DBA(a copy is attached)
1.
Her insurance (bond) policy has been expired since March of 2.
She is falsely telling all of her students they are bonded through
her.a.
I called the number for “*** *** ***” because that is
who the insurance was “administrated by”b.
They transferred me to their “online program” which was then
identified as *** * *** ***.c.
Dave was the gentlemen that was helping me.d.
I asked if this policy # and certification # was still active and he
said noHe then told me he was looking up her name to see if a different
policy was taken outI was informed by Dave “no, she does not have a current
policy” with them but he would not know about another company that may provide
it to her!e.
This was the policy/certificate that she showed me the day of training
in her homef.
This policy/certificate does not nor does this insurance company deal
with the “MEDICAL BOARD” that she keeps talking aboutIt is for Beauty Salons
and she is not a Licensed Beauty Salon (which we know has nothing to so with eyelash
extensions)So now we need to know if she is insured through the medical board
for her medical grade equipmentg.
I have contacted the Ohio Medical Board and you do not need a nurse or
doctors supervision either h.
On the “A & B” of her policy, it is not specifically stated
“eyelash extension” as a service provide by the insuredAnd because both the
health and beauty industries do not recognize eyelash extensions and a board
licensed service, it would need to state she is specifically insure for eyelash
extensionsMy insurance company has me update annually what my specific
services are that they are insuringThis class is fast paced so if you're talking while I'm talking then
you may miss some materialEveryone is teaching the same information nothing
has changed other than the fact of me adding a years time to so that you can
learn more techniquesThis class was by no means fast paced unless you consider her being a
half hour late to start and then an additional hour late after lunch, That would
make HER have to rush through the class P.S
This is the last response that I will giveConsidering the obsessive
compulsive reactions that I am getting not just from Revdex.com but others they all
have dismissed her claim as frivolous because she agreed and new of all the
private training in advance.Not
sure at all what she is talking about hereThe first and only complaint is the
one we are dealing right here with the Revdex.com! I
am a little confused as to her current response because it is not addressing any
of the issues that where brought to the Revdex.com and her attentionKeeping her
current response in mind, Please understand that any of the issues that where
previously discussed, she is not addressing here any longerSo now we are
dealing with new issues that she had brought to lightSo in finding out today,
by doing my research, that she is also no longer insured for the schooling and/or
services that are being performed and the students under her that are
performing these services, all of which are in her homeIt
has been proven that she is lying time and time again, including my current
response with the insurance issue!1.
She has lied about the class start time and the time of phone call
that I made to her2.
She has lied about having to be “bonded”3.
She has lied about her own bonding4.
She has lied to each one of her students since March that they
are bonded under her5.
She has lied about us not being able to do this service in the salon
saying state board would not approve itI received a call from the Ohio State
Board of Cosmetology and they informed me that the service IS allowed to be
done in salons with the proper education/certificationIt is not inspected by
them but they do make sure you are following proper sanitary guidelines of the
board of cosmetology.6.
She has lied about needing nurses or doctors supervisionThis is not
required through the Ohio Medical Board, nor is it recognized with them as
well.7.
She has lied about us using medical grade equipment, the tweezers that
she is claiming to be medical grade equipment are sold at professional beauty
supplies, I am sure they are sold for medical use as well, but it’s a little
stretch of the 8.
We were deceived as to where the class was heldAll
advertisements/website said *** *** and it was at her home.9.
She has lied about the contractIt was never completed by her nor was
it signed by me We
still have no idea what we paid tax on or for!

I have reviewed the response made by the business in reference to complaint ID ***, and have determined that this does not resolve my complaint. For your reference, details of the offer I reviewed appear below

In response to Sarayah A*** response on complaint #
***. (Sarayah’s comments are in small regular font and mine are in bold
italic.)
Message Includes contract sign by trainee
Contract was never “signed” by myself nor the TrainerEven in the
copy that she sent to Revdex.com, the contract was never completed by Sarayah
1.
Trainee has three choices regarding where they are comfortable
training such as online, private, or at the *** ***
Yes we were offered the private class because we could not make the
Wednesday or Friday ones that she offered and allowed us to choose a day,
however it was never mentioned that it would be in her homeEverything that we
read prior to booking the class said it was at the InterContinental Hotel Please understand that it being in her
home was not the bigger part of the issue until we had to deal with the
unsanitary conditions (her son touching our “medical grade equipment” and a cat
being in the premises)We did not purchase through Groupon, but that is where we found her and saw that it is in a hotel, we did not realize the addresses are different in the confirmation emails, just assumed the hotel{ex: F 1-4}
Lashtown Cleveland is not nor has never implied that they are a
school
There is reference on the web directing us to her website of Lashtown
Cleveland being a beauty school on {ex: A} that was emailed over
Lashtown Cleveland has never stated that they are registered with
the board of cosmetology nor implied such; due to the fact it is not regulated
as such and we are using all medical grade equipment
I am well aware that none of this is concerning the Ohio State Board
of Cosmetology, that is why my complaint was filed with the Revdex.com and not the
Ohio Board of Cosmetology regarding our lash trainingThe only reason we
mentioned and looked into it (after the fact) is because she is offering salon
services such as facials which require you to be licensedNot relevant, but
also doesn’t sit well with me And to address the “medical grade equipment”… again with the
unsanitary atmosphere
Trainee signed a binding contract stating that they would not do
such a thing
Not understanding this one fully… “such a thing”… as wanting our money
back for inadequate training… then I am glad that the contract was never
“signed” by all partiesI also have sent over a copy of my contractIt looks
exactly like hers… not completed with any signatures.{ex: B 1-2}
.Advised trainee that she has ongoing support for one year after her
certification and can receive additional training as needed with no additional
cost to her She was also informed that she may still attend the class at the
hotel is she choose but will need to bring all learning materials with her and
money for food and parking
As I do appreciate the gesture, I will repeat what I stated in my
original complaint“I am not willing to drive another hour and a half to get
the same possible inadequate training” (and that is only one way; hour drive
total)
Private training sessions are at 1030am until trainee completes
full set Trainee arrived at 9:15am
emails are also attachedThese emails were sent to me confirming the
time of the class and both emails state 10:00am{ex: C 1-and D}
I also stated in my original complaint that we arrived at 9:42am and
that was because I called her to make sure we were at the right place… I am not
trying to call anyone a liar but I do have the call log and my co-worker to
prove itA copy of the call log has also been sent over {ex: E}
Due to our binding agreement and all that is already being offered we
will not be issuing a refundThe next class at the hotel is on October at
am
The request for the refund is so that we can use it to get a reputable
company to certify us properly
*** end of response***
I would also like to address a particular part on the contract to
explain the “inadequate training” that I am referring toMost, but not all, of
this below is in the stapled training packetSome may have been reviewed
verbally and some is there for our reading later (meaning she did not go over
it in class)but the training packet does not have all of the information
stated in the section below labeled “This hands on training includes the
following” other than physical hands on partExample, “State laws and local
rules” is not mentioned anywhere in the training packet it was just briefly
verbalizedBelow is "This hands on training includes the following" is a section from her contractMost of which was
either not addressed at all or only partially addressed with my explanation
below:
1.
Diseases and disorders; YES… it was gone over… she read from a piece
of paper and commented that she didn’t even know what some of the stuff was
2.
State Laws and local rules; YES/NO… we were informed that this has
nothing to do with the Ohio State Board of Cosmetology which would be rules
and/or regulations, other than that no other “laws” were brought to my
attention
3.
Bonding requirements; YES but a lie … we found out through other companies
that this is not even a thingHowever, we were told by Sarayah that we had to
have it and now we were “Bonded under her” Why would I go get training to have
to work under her (an hour and a half away)I am aware of other people that
have been certified in this service and they work for themselves. She did tell me how to get bonded on my
own, but again, after talking to other companies and certified technicians in
the field this is not even a thing!
4.
Product knowledge; NO… She never went over the all of
products in our kit nor explained what and how to use “primer” which is
essential and was in our kitsShe did go over a few chemicals that may be
found in some products but, if you consider her telling us to go to certain
websites to buy product…Then OkBut, the kit we received, she never went over
all of the products inside except to say… “feel the lashes” and “make sure your
kits look the same”
5.
Introduction and understanding natural hair growth; YES… that information
was read to us
6.
Client Consultation; YES… that was gone over
7.
Framing and eye design; YES… that was gone over
8.
Proper workstation setup; NO…Setting up a “workstation” was not
gone over nor shown to us
9.
Client preparation; NO… This did not happen… because of previous lash
training that I have had, I know that the “client preparation” was not gone
overEven before my co-worker and I started working on each other, the
instructor did not have either one of us cleanse or prime the eyelashes
10.Cleaning; YES… a little
talk about the use of alcohol and using medical grade disinfectant
11.Tools and use; yes we did work with
our tools
12.Fills and Maintenance; YES… it was
talked about (hard to demonstrate until you would have a client return)
13.Removal; YES/NO… this
was demonstrated briefly (not a full removal though) not to hard to figure out
14.Aftercare and
maintenance; NO…we were never told how to inform the client how to maintain their
investment
15.Waiver and release form; YES… this was addressed

I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below.
Regards,
[redacted]
In Sarayah's response to my complaint1. "Trainee has three choices regarding where they are comfortable
training such as online, private, or at the [redacted] .My coworker and I did book a private session, but it was never mentioned in any form of communication that it would be at her personal residence. Her website [https://sites.google.com/site/lashtowncleveland/home] and also her StyleSeat account [https://www.styleseat.com/lashtowncleveland?q=lashtown+cleveland&pref=searc... both state that Eyelash Extension Training will be located at [redacted]. 2 . "Lashtown Cleveland is not nor has never implied that they are a school .When you search 'Lashtown Cleveland' on [redacted] or [redacted] search, it comes up labeled as a Beauty School. Which is very misleading. Her home address is labeled as the 'Beauty School' address in these searches. (reference in attachments)3 . "Lashtown Cleveland has never stated that they are registered with the board of cosmetology nor implied such; due to the fact it is not regulated as such and we are using all medical grade equipment .On both Groupon Sarayah labels herself as a Salon (reference in attachments). On StyleSeat, Sarayah claims herself to be an Esthetician(reference in attachments)- which are both licenses given by the Ohio State Board of Cosmetology though she has no record of receiving a legitimate license according to the Ohio License website. As I know that Eyelash Extensions are not regulated by the Ohio State Board of Cosmetology, you can not claim yourself to have professional credentials without having those credentials.  No matter whether 'medical grade equipment' or not; these services need to be practiced in a sanitary environment. 'Medical grade' or not, there was no place for us to sanitize our tools. Sarayah demonstrated bottom lash extensions on both my coworker and I, without sanitizing her tweezers in between.  4 . "Trainee signed a binding contract stating that they would not do such a thing"I never received an emailed contract nor a copy in person with Sarayah A[redacted] signature on this contract. Even in the copy that I was forwarded by the Revdex.com- she still didn't have her signature and I never signed my name either. Also in the contract that we were given, her legal name is stated as "Sarayah M[redacted]" which causes some confusion. In her contract that she had sent us there were many expectations of what we would be learning in the class that I did not feel we touched base on or learned adequate knowledge on. Such as State Laws and Local Laws/Bonding Requirements; the information that was given is that we are required to purchase a medical bond per individual lash stylist to perform these services- we found out that this was not true. We were also told that you are only allowed to perform lash extension services in a separate room aside from any other salon services. This is also not true as Eyelash Extensions are NOT regulated by the Ohio State Board of Cosmetology.Product Knowledge/Client Preparation; She touched base on a some types of glues and lashes that she prefers, also went over some of the chemicals found in different types of glues. She never went over products used for cleansing the eyes or prepping the lashes. She also never demonstrated on the mannequin, myself, or my coworker how to properly prepare the eye for a lash service. Tools and Use; As I stated in my original complaint we were not instructed on when/how to use a majority of the objects that came in our kits. Proper Workstation; Neither during the mannequin nor the live model application, were we instructed on how to setup a sanitary workstation. We had no tools out other than glue, lashes, tweezers, and a finger cup to hold our glue. Aftercare/Maintenance; We were never instructed on any at home care to advise our clients on how to maintain their lashes. I did not receive adequate training in multiple areas of this 'contract' and request a refund to be able to go to a reputable Lash Extension Training class to be able to learn the proper and safe way to perform these types of services.5 . "Advised trainee that she has ongoing support for one year after her certification and can receive additional training as needed with no additional cost to her . She was also informed that she may still attend the class at the hotel is she choose but will need to bring all learning materials with her and money for food and parking."After I left a day of training with 'Lashtown Cleveland' not knowing how to perform a full lash extension service, I would not want to waste my time driving from [redacted] to Cleveland, OH on my expense to receive another day of insufficient training. I also do not want to put myself in a position where I am practicing lash services on a live model in an unsanitary/unsafe environment once again (as I stated before there was a cat present roaming the house and also her small child being on the massage table during hands on training and touching our tools).  6 . "Private training sessions are at 1030am until trainee completes full set . Trainee arrived at 9:15am"I received a confirmation email on 9/19/2015 station that my training was booked for Monday September 21 at 10:00 AM. (reference attached in files)[redacted]
