Patient Advocate/Risk Manger [redacted], who I have copied on this email and LGH was found NOT to be at fault for the damage to Mrs. [redacted]'s teeth.   The dentist confirmed that "the fracture of this nature can be consistent with a traumatic event", which does not happen during surgery without documentation in the patients record.  There was not documentation in the medical record to indicate that there was a traumatic incident during surgery. If you have any further questions or responses, please feel free to contact [redacted] at [redacted].[redacted]@lawrencegeneral.org as I no longer work in the same capacity for LGH.  She can further assist you with any complaints. Thank you for our help with this matter. [redacted]Community Medical AssociatesAmbulatory Support Services Manager[redacted]
