April 5, 2016Dear [redacted],On October 27, 2015 G) 1 p.m. we were contact by [redacted]'s Hospital's social service department to transport [redacted] from [redacted]'s room [redacted] to [redacted] Rehab Hospital on [redacted] in Allentown. At no time did any of our staff quote [redacted] a price, that...

may have been done by the staff at [redacted]'s who arranged his transportation. Being we offer a discount to people we transport out of [redacted]'s Hospital, [redacted] did see a discount on his first bill of $60.00. Also noted clearly on his invoice was that if not paid in full within 45 days, the discount would be removed. On November 31, 2015 we received a call from [redacted] asking us to bill his insurance, [redacted], which we did on January 8, 2016. On or about January 27, 2016 we received an EOB (Explanation Of Benefits) from [redacted] (attached) showing they were NOT going to pay this claim as a non-covered service. Another invoice was sent to [redacted] stating his insurance does not cover wheelchair van transports, and he is responsible for the bill. We received a call from [redacted] stating [redacted] paid his bill. He was more than rude and was using foul language during the call. After he insulted myself and our company did become rather firm with him and explained again have a copy of the insurance denial in my hand, it was not paid. We then sent him a copy of the denial with his next invoice, however he was well past his 45 day period and the discount was removed. He now owes us $123.75.We have been nothing but fare and honest with [redacted], and he has been rude, nasty and uncooperative with us. Review: We have flight crews sent over to us by this taxi company and on the night of the same night 3/29/2013 the taxi fare was $12.55 one time and $20.00 another time during the same day. The next day the fare was back to $12.55. They appear to be changing the rates to whatever they feel at the time the service is provided.Desired Settlement: I have contacted them on a few occassions and I have not received a call back to explain this to me.
