I have had the opportunity to do a thorough investigation on your account. These are my finding: ·         Consumer dispute via [redacted] received on 05/23/16 ·         SCS responded via [redacted] on 05/31/16...

·         Consumer requesting Verification via [redacted] received on  06/22/16 ·         Consumer sent Certified Letter #[redacted] on 06/29/16 ·         SCS requested additional documents from Original Creditor 06/29/16 for proper review. ·         Original Creditor (SCS’s Client) Request Account to be Canceled 07/01/16 to review [redacted] ·         SCS sent Consumer Zero Balance Letter and Cancelled Account letter on 07/01/16 ·         SCS Requested Account be deleted from [redacted] on 07/01/16 ·         [redacted], has up to 30 days to Delete the accounts. This is an [redacted] rule that SCS has no control over. ·         All 3 of Consumer’s Accounts were closed as of 07/01/16 I understand your frustration on trying to resolve your accounts and I appreciate you sending the Certified Letter and providing the additional [redacted] Information. It was because of the information you provided that we were able to resolve the account and Close the accounts on July 1, 2016. Unfortunately at this point we have responded to all of your communications in the appropriate manner and within all legally required timelines. I do understand your frustration about the account(s) still being on your credit. As such, we have sent a second request for your account(s) to be deleted from [redacted]. As requested by the consumer, the document that was signed by her when she consented to treatment is attached for review. This was for inpatient procedure at Cypress Fairbanks Medical Center Hospital ER and Imaging Center beginning 09/23/14. This document was obtained from the original creditor. The consumer is welcome to call 281-933-1355. Any of our Patient Account Representatives can give her more specifics about her account. If she would like, the consumer can ask specifically for Jane [redacted] Jane will be happy to be her personal Patient Account Representative. Thank you.

time of the request, except that- if the applicant to whom the file relates requests that the first 5 digits of the social security number (or similar identification number) of the applicant not be included in the disclosure and the CRA has received appropriate proof of the identity of the requester, the CRA shall so truncate the social security number in such disclosure; and 2. The sources of information; except that the sources of information acquired solely for use in preparing an investigative consumer report and actually use for no other purpose need not be disclosed. However, in the event of a lawsuit, the sources must be made available to the plaintiff under appropriate discovery procedures in the court in which the action is brought. 3. Identification of each person that procured a background check for employment purposes, during the 2- year period preceding the date on which the request is made.  This was competed with a letter to the consumer, as she stated in her original complaint to Revdex.com, on 0[redacted] This letter met all requirements as outlined by the FCRA. Although she is asking for “Original” documents, we have no legal requirements to provide that to her, or any consumer after the Validation Period has passed. The Validation Letter was sent on 04/12/2015. Collection agencies are required by the FDCPA to give the consumer thirty days to dispute the debt. If an account is disputed during the Validation time period then a complete verification would be conducted and proper documents returned back to the consumer.  The validation time period for this consumer ended on 05/12/2015. As a curtesy, we did provide additional documents and copies of documents in our letter to the consumer on [redacted]  I understand the consumer’s frustration as medical bills and all the governing agencies (FCRA, FDCPA, etc,) can be complicated and confusing. However, at this time and after a complete review of the account, we have provided the consumer with all documents that are required. We are willing to work with the consumer with many different resolution options.

to you. All of those are listed as disputed. We do have one account which we received a letter from the guarantee in November of 2016. The correspondence indicates there was ID theft however does not list medical or insurance fraud. In addition the letter from November 2016 did not provide a date of birth or Social Security Number and therefore we were unable to verify the correct account was being addressed. I am happy to close all your accounts that may have been caused by fraud and or theft. Please call and ask for Joan, she is going to help you resolve these accounts. We will need proper documentation from you in order to close these accounts correctly and properly. Joan will be happy to help you resolve your accounts.
