Received a letter of a collections balance in July 2020 for an ambulance service in January 2020. The date of service in question was a fraudulent issue and was filled with a police report. When contacting Life Line Billing Systems 7/17/2020 I spoke with Rep. *** and was provided a fax number to send my police report. The paperwork with Hales Corners Police report was faxed the following day. Following up I was verbally told that this matter was resolved. On 9/4/2020 I received a letter from Wisconsin Department of Revenue of an income tax intercept by Life Line Billing Systems. I have called multiple times and assured a manager would promptly return my call, which I have not. Given that they were provided a Police report back in July there should have been no reason for this company to hold my tax return without my permission. My next course of action after giving the Revdex.com an opportunity to assist with a swift return of held funds, will to cause me to take legal action.

Regarding Revdex.com complaint #*** received 9/10/20Contact was made with the customer via telephone on 9/10/20. We explained to the customer that wehad not received the related police report which clarified the disputed amount. He forwarded our officea copy of the police report via email on 9/10/20. The account was then closed due to proof of fraud.Refund checks totaling $639.00 were sent Thursday, September 10, 2020 via overnight mail to thecustomer. My wife was taken to the hospital with an ambulance. The bill from the ambulance company was above 90% of the usual cost, so my insurance company did not cover the whole bill. It is absurd to me that they charge so much.

We have reviewed the complaint by Mr. regarding the amount charged by our client, Eau Claire Fire and Rescue Department (ECFRD). ECFRD does an excellent job providing 24 x 7 life-saving services. ECFRD’s primary concern is the care of the patient and making sure their needs are taken care of.
Costs of the ECFRD are only partly funded by federal government programs, private insurance and the patient/insured party. The costs are also subsidized by the local government. Billed rates for this service are in accordance with the rates set by the City of Eau Claire for the services performed.
Each year the City of Eau Claire reviews the charge rates of the ECFRD compared to other EMT service providers of similar size, geographic area and the level of care provided. ECFRD’s charges are significantly lower than most other EMT service providers in northern Wisconsin.
Lifequest Services role is to invoice and collect on the services provided. The insurance provider receives the invoice and then processes the EMT claim. The sharing of the cost relating to the claim is allocated between the patient/insured and the insurance provider, pursuant to the insurance contract. It should be noted that of the bills we process, many insurance companies cover the full charges.
Local data and other insurance companies paying similar charges indicate that the ECFRD rates are not excessive. Mr. can work with his insurance company regarding their approach/partial nonpayment for services.

[Can you please provide data that tells me that about the set rates? Right now, I just have an email that says the rates are significantly lower than most other EMT service providers in northern WI. I need some data to go back to the insurance company to ask them why they did not pay the full portion. ]

They intercepted my tax returns and I have told them they have the wrong person and they agreed to return the amount, but have failed to do so.

I included you in the initial response to the complainant. We have overnighted the balance due plus the additional admin fee charged by the state. His credit reporting has been cleared by LLBS and all other personal records have been purged from our system.

Unfortunately this situation should not have happened and was a result of human error. We have since dealt with the personnel part of the equation and added additional controls to prevent further issues.

The customer contacted us on multiple occasions which should have escalated the issue internally - it didn't, that has been addressed as well.

In 2015 I was on vacation in Wisconsin Dells at the Wilderness and had EMS's called to the pool for my friend. After they arrived I then passed out and they assisted me. They never transported me or asked for any insurance information. In june 2018 I turned 18 and applied for a credit card, was immediately declined because of poor credit. I thought it was weird because I have never had a loan or credit of any type. I went on credit wise to see why my credit score was so low. I then saw that I owe $277 to Life Quest Services of Wautoma, WI 54982-5267. I called and talked to Kay H. Foret and asked what this bill was for, she told me it was for services from Dells-Delton EMS. I then asked her why it was on my credit and not my parents. She then told me she doesn't know and said they have been mailing me a bill for 2 years and 11 months. My family and I have never received any type of bill from this company. She stated that they are only the collections office that doesn't have much information but that I was seen and have this bill. I requested a copy of all details regarding this bill. I have waited for 3 weeks now and still have nothing. I called them back on January 31 2018 and asked to have it emailed over. I received a email of just stating my bill there was no other information. I also asked if I could call the people this originally came from and she stated that I would just be sent back to her. They then told me since I have been in collections for so long that they will take it from my tax return. I do not understand why this is my credit not my parents because I was a minor. Since my parents or the insurance company has not received any information of a bill from Dells-Delton EMS. They could not even tell me my address nor my parents names.

Included below and attached are documents we forwarded to Hailey Kuhns regarding the above mentioned complaint. We apologize it has taken a little longer than anticipated to respond to this complaint however, we wanted to positively verify that the records have been deleted from her credit report. Unfortunately it had taken until yesterday for that verification.

The original debtor and responsible party (parents) had been informed of the balance due and they followed up with insurance requirements (document attached). We subsequently submitted the insurance, which was applied to the deductible leaving the balance still owing (document attached). We had followed up with letters notifying the debtors of the balance due, the balance went unpaid for a period of time. We were recently notified from the complainant of the credit reporting concern and immediately put the account on hold until the issue could be resolved. We have subsequently removed the credit reporting filing and have received verification from the credit reporting agencies the records have been deleted.

Hopefully this will positively resolve this matter. If you require additional information from LifeQuest, please feel free to reach out to me directly and I will provide whatever further clarification you may require.

If I am correct, you verified that the collection amount was removed from my credit report. What I do not know is if they removed their request to take my taxes. Following-up regarding your most recent response to the above-mentioned complaint. Your Tax Returns will not be affected by this claim. Any reference to Tax Intercept has been removed from your file. Hopefully, this resolves any open concerns you may have and if you have any further questions, please feel free to let me know.

I received billing from them for an ambulance ride to Mayo Clinic (I was having a heart attack). Insurance did not pay for it. After receiving the bill, we agreed mutually on a repayment of $25 per month until paid. I have been paying that amount for three years. Without warning, discussion or negotiation, Lifequest one day decided they want the bill paid faster and began billing me $46 a month. I responded to them that it was not agreed to by me to pay more than what we mutually agreed to and I would continue to pay $25 a month, they continued to send me bills for $46 a month. I dispatched a letter to them saying that I would pay the agreed to amount, not $46 a month. They then called me and told me I will be paying $46 a month. In my circles, that is breach of contract, and will now file a complaint with the office of the Wisconsin Attorney General as well as the *** District Attorney. I did not agree to pay more, I am not going to pay more because they demand it. I expect Lifequest to honor the contract they have with me, or I will need to take steps to make that happen.

We have reviewed Mr.’s complaint and his account.

This account began in 2014. Attempts to reach Mr. in 2014 and early 2015 to set up a payment plan were unsuccessful. Mr. began sending in $25.00 payments in February 2015 and although these payments were below our standard payment plan we processed these payments. We show no formal agreement creating a $25.00/month contract. This arrangement of $25.00/month resulted in a 4+ year payment plan, without interest; far exceeding normal billing standards, but we did accept this arrangement by invoicing him accordingly.
Mr. has breached this payment plan numerous times since 2015. Payments were missed in December of 2015, January 2016, May 2016, November 2016. Several times he was set to go to collections, but avoided by sending in another payment before being forwarded to them. Two of the four times this included a double payment.
In December of 2017 his payment was again late and we sent him a another notice stating payments need to be received every 30 days to stay in good standing. At that time his account was reviewed by our billing staff and determined to have been non-compliant with his terms of the payment plan five times over the course of this arrangement. Accordingly he was sent a notice increasing his payment to $46.00; still a reasonable payment amount considering the number of defaults and timeframe this account has been open.

In regards to the complaint and the list of ‘talked to company’ dates. We show no record of calls on 1/3/18, 2/5/18, or 3/22/18.
A customer care representative did talk to Mr. on 04/11/18 regarding the $46.00 payment plan.

In good faith we would agree to once again lower his payment to $25.00 / month until fully paid off with the understanding that the next payment reaching default status (missing due date and 15 day grace period) the file with immediately go to the collection agency.
