Hi,

size="2">Heart patients always have a more complex time of obtaining life insurance as heart disease is #1 cause of death.
In this case, we only obtained heart ablation procedure records on Dec. 1 and immediately forwarded them to [redacted] for consideration.
[redacted] will not make a final offer until they have first reviewed those records.

Here is a diary of our service record on this case:

The current status of this is that [redacted] is reviewing documentation that I sent to them on 12/1 and deciding whether or not they will reopen the case and proceed.

Here is a quick breakdown:

07/2106 - [redacted] app written by [redacted] was rated by [redacted] due to history of Afib. It looks like he did not like the price so pivot to [redacted]/

7/22 submitted to [redacted].

8/9 At best offer from [redacted] processed and reassigned to Peggy. reason: History of asthma treated with Nebulizer at home in 3/2015 as well as heart murmur and palpations.

8/18 Customer told [redacted] he would proceed and [redacted] requested  medical records.

9/2 APS was received but [redacted] said they wanted the request reopened to obtain details regarding an ablation procedure he had done.

I wanted to speak with him about this and make sure we were loo[redacted] in the right place.

9/21 Left him a message

10/3 spoke with him

10/11 left a message and sent an email as[redacted] for assistance obtaining the records that [redacted] was as[redacted] for.

10/19 left a message informing him that I needed the records or I would have to close out the file

10/20 emailed again as I did not hear back

10/20 Heard back from him and told him that [redacted] needed any records pertaining to the ablation procedure.

11/3 Spoke with him and he said he was going to call his Dr's office and try to get the records about the ablation procedure.

11/9 Spoke with him about an authorization form to release the records to EMSI as they were still trying to obtain the records as well.

11/17 I asked him if he was having any luck with getting the records and let him know that I was closing out the file until they were obtained.

12/1 Records were sent to [redacted] by EMSI.

12/16 I contacted them and asked for an update from [redacted]. They responded that they needed some time for the underwriter to review and decide whether or not they would reopen the case.

12/19 He asked me if there "any way to get up with [redacted] on the status of this app?" at which point I told him that I was waiting for [redacted] to get back to me.

Sincerely,
[redacted], Founder and CEO

Given the medical history of Mr. [redacted] and his unwillingness to work with us in a civil manner, we know of no other life insurers that would accept him.
