review of Mr***' medical record has been conducted regarding this complaintUponreview, DrMaritza *** confirmed that all LHMG provider interactions with this patient were clinicallyappropriate and met community standard of care.LHMG staff have spent considerable time speaking with Mr*** including the *** *** ***Practice Manager and the Compliance Manager to better understand his concernsLHMG has attempted toconvey our expectations with Mr*** related to his care and treatmentIt was explained to him thatoffice visits are required before specific medications may be prescribed and/or to monitor general healthMr.*** did not agree with the documentation in his medical regard related to clinical diagnoses made byDrMaritza ***All communications from Mr*** regarding his grievances and concerns related toLHMG are included in the communication section of this record and serve as proof of his concerns.Mr*** was discharged from LHMG effective May 9, under the patient discharge policy criteria ofnon-adherence or refusal of patient to participate in the agreed upon plan of care or treatment plan on two ormore occasionsLHMG included a medical record release form to change providers with the discharge noticeand we will accommodate his request to transfer his records at any time.LHMG is committed to providing excellent service and quality care for all of our patientsWe regret that Mr.*** does not feel we have met his expectations.Sincerely,Michelle ***, Compliance ManagerLifetime Health Medical Group
