MAP [redacted], Eff. 10/05/2013 to 10/05/2014:Surcharge from the9/15/2013 accident was vacated by DOI Board of Appeal on 7/31/2014. Endorsementsprocessed 8/18/2014 & 8/20/2014 to amend policy to remove all SDIP relatedsurcharge totaling $416.00 in credit. We have determined this is the correct...

credit.Refund check for $416.00 send to insured on 9/03/2014. MAP [redacted] was renewed to AMV [redacted], Eff. 10/05/2014 to 10/05/2015:Endorsements processed8/18/2014 & 8/20/2014 to amend policy to remove all SDIP related surchargetotaling $426.00 in credit. We have determined this is the correct credit.Since the endorsements were processed prior to any billing on this term, thecustomer was never billed nor paid any premium relating to the surcharge on this policy term.We do not see any other adjustments we need to make to either of the insured’s policies. In regard to hearing fees the insured paid, it is at the discretion of the policyholder toincur that expense and we do not reimburse a policyholder for those fees.

We have reviewed the customer’s policies a second time. The policies are rated correctly based upon the information provided to us. We do not see any adjustments that are required on the customer’s policy at this time. We believe we have met our obligations to the policyholder.
