Per our conversation, it is policy that our PM & R physicians (Physical Medicine &...
Rehabilitation) do not complete disability forms for patients. They are referred back to their PCP (Primary Care Physician) or Family Physician to obtain any disability that is given. Our Medical Records Department will and has provided the [redacted] with Mr. [redacted] medical records that have been requested. This policy has also been communicated with Mr. [redacted] directly by both our Medical Records Clerk and also the physicians Nurse.
