he is referring to also states that the co-pay is due at the time of service. The 20-50% he is quoting means the copay is 20-50% of the total fee. This may be the part that was confusing. Our office policy is clearly spelled out that we collect the full co-payment at time of service and we do extend the courtesy of filing the insurance claim forms. The balance due after insurance has paid or denied is also the responsibility of the patient. And the quote/treatment plan is simply an estimate with no guarantee of payment by the insurance company. We are happy to produce the forms, if needed. There is always a manager on duty to speak to our patients. Thank you.

Basically, when I asked twice to speak to whomever was the manager involved, I was told both times different versions of "There's two of them back there and they don't speak to patients.
If anyone would've been clear to me that there was a misunderstanding on my end and explained it to me, I would readdress this entire issue.
I have the form where they literally scribbled that down payment section out and gave it back to me.
At this point, I've invested too much time with this.
