Review: I have been unable to obtain accurate invoices for services rendered and am unable to file insurance claims for the services. I have asked for invoices on several occasions which must contain dates of services, charges incurred and the company letterhead so that I may file for insurance claims. The following is the last email where I state what I needed from Lisa Schlesinger:Good Morning LisaI received several versions of the bills you prepared. I am referring to the spreadsheet you sent that has the breakdown by month and day, which has a balance of $2281.34. This of course does not match the balance you sent in a [redacted] delivery which shows I owe $2141.34. I would like to get this cleared up as soon as possible, however the statements are still not accurate. I have the following comments on the spreadsheet you prepared earlier, the one with all the months and dates of visit.1. I would like the spreadsheet to have your name and address on it for submittal to the insurance company2.In April you have appointments 5 days a week (15th thru 19th and 22nd thru 26th) which is not correct as I never had appointments all 5 days in any given week. In April you do not show appointments on the 29th and 30th. Again, if the appointments were Monday through Thursday, there should have been appointments on these two days. So the total for April would be the same if you show the 29th and 30th and delete the 19th and the 26th.3.In May, I believe I was coming in Monday through Thursdays so I was not there on Friday the 3rd. Also, I was never coming in 5 days a week as you indicated on the sheet. During that month we changed the appointments to Tuesday through Friday instead of Monday through Thursday so I did not have appointments on May 6th, 13th, or 20th. The fee for May 30th was not $150 but $30. Therefore there should be a total of 18 visits in May for $30 each or a total of $540 in May.4.The month of May was your last month with the insurance. So my check paid on June 5 for $840 was for the $540 owed for May plus two visits in June for $150 each (the 4th and the 5th of June). 5.For June, you show an appointment on June 3, which did not occur, because again, I was only coming 4 days a week not 5 and you show 5 sessions that week. The total for June should therefore be $2,400 not $2,550 which makes sense because I wrote a check for $2,400.6.I think the dates in July are correct. 7.In August, you show an appointment on the 19th which is correct and that is the day you said the check bounced and I did not continue the session. I told you on that day that I could not do therapy anymore indicating to you that I was finished with therapy. Therefore, for August the total bill is $150, not $750 as with the notice, I am not obligated to pay for appointments after the date of termination. I have no written agreement with you to pay for appointments scheduled after termination and you did not provide services during those dates and therefore you are not owed for those dates. The total due in August is $150. By the way you have 5 days in August. I never had appointments 5 days a week ever.Therefore, with the changes above, the total amount for all services is $8,280. With the credit you offered of $1,108.66, the balance is $1,291.34. I can send a check for this amount on Monday if you can provide the corrected statement as requested above.Desired Settlement: I have not received accurate invoicing from Schlesinger in order to file insurance claims. I have received several versions of invoices each different. I have asked Lisa to communicate with a third party biller and she has refused to transmit needed information to the billing company and therefore I cannot file claims to insurance. If I cannot file claims to insurance due to in action by Lisa Schlesinger, I will ask that she refund all but co-payments so that she may get reimbursement herself.
