The live right senior care was hired to assisted in my fathers needs due to in-stage copd
. On 11/2/2019 father Thomas L.*** was released from a nursing home location *** south Halsted from recent event of problems breathing ( father is deceased in 2/4/2020 ). We discussed the possibilities of being hired from 10/16/19 -10/30/19 . The decision the contract of was signed to started immediately. The agreement of contract was simply to live up to their duties on level 1 plan in which laundry , bathing, light cleaning such as vacuuming washing the dishes , errands medical dr or medication pick up , meals preparation —— and that each caregiver have a car for sudden need of transportation—— working 12 hour shifts from 5:pm to 5:am Mondays through Sunday’s . That there would be 1 caregiver because father retired living in fixed income / the Director Sandy *** insisted that oh it will be 2 caregiver to offset the charges of overtime by 2 caregivers one would work 4 days the other would work 3 days so they could alternate off days as well —— the first week one of the caregivers worked for 7 days straight a check was handed to director of 2980 of calculation of the actual working days for both as and deposit of 12 hour shifts for 24.00 an hr and 25.00 on the weekend agree . As the weeks develop it was noted that in 11/17/19 a view of medication which they could give but placed in container by me and another homemaker I Stephane daughter hired to assist in the 9-5pm shift in which Waylin did all the preparation of meals; all the washing of dishes and it was never caregivers doing completed job of anything and medication was removed from containers purposely by Bronsette Horton a caregiver who came later about the week later ; making calls at 4am that was not necessary and inappropriate behavior ; sexual enticement ; sleeping on the job; cheating time sheet for the termination of service on 12/13/19 they kept deposit/ under contract and debt father acct without authorization by father or me.
