  I asked Livingston podiatry for dates of service and what I had performed on each date on a copy of their letterhead and I was failed to be provided that information  and I was spoken to very rudely by an office staff member

The complainant called several times for copies of her office visits so that she may submit to her insurance carrier. With each phone call she was instructed to call ProHealth, as we did not do the billing nor did we have these records to provide to her, we were a part of an organization that did...
