February 14, ID [redacted] / [redacted] phoneDear Revdex.com representative [redacted] We have provided all records, progress notes, and billing statement information which you have requested for [redacted] ***As you can see from our records, everything that was on the treatment plan that Mr [redacted] decided on was completed October 21, 2015, a year and months agoMr [redacted] was given treatment plan options when he originally consulted with our officeAs you can read in the patient record, which is a legal document, he had the option to fix the existing teeth prior to making the lower partialFixing the teeth prior would ensure that the teeth would not further degrade, hence is a longer and more stable resultMr [redacted] was informed and understood this, but made the decision to only have the new lower partial done at this timeThe custom metal framework was made to fit his existing teeth, and it fit perfectly and was satisfactory to Mr***As the teeth abfract on the side from wear, the partial does not change, and therefore looseness occurs.Building up the teeth after the custom framework would lead to the partial not fitting at allInitially we were asked by Mr [redacted] to fabricate for him a maxillary denture and a mandibular partialWe fulfilled our obligationRestorative work on his own teeth was offered, but was decided against by Mr [redacted] and of course was never billed.I have been practicing dentistry for years in this community and have seen thousands of patientsI have never had a claim with the Revdex.comThere are times in our dental field that treatment does not always go as planned, but in these cases we make things right with the patientHowever, in this case Mr [redacted] received and paid for just what he requested, and the hardship he is claiming arose from his personal decisions.Thank you for your consideration.Sincerely,LKevin M [redacted] , D.M.D

8-4-initial exam-was given options? A.to put crowns over existing teeth permanently? [redacted] didn,t want anything per ? anent over old worn out teeth in case they would deteriorate, plus he couldn’t see paying $10,for something that didn’t seem like a good idea? BDrLKevin M [redacted] said he was going to build up remaining teeth at a later date so partial would fit snug? [redacted] chose this option? This never happened? My wife***,also heard this offer? I’m Years old ?" double amputee ?" diabetes ?" am hard of hearing and I always talk loud ?" not being unstable except I don’t want to be taken advantage of? I’m not able to eat with my lower plate in? I’m afraid of swallowing it? I paid $for nothing? I was upset when I wasn’t going to get what I was promised? Instead I was dismissed from their practice

right cuspid #and a single tooth #All remaining teeth had excessive wear from the facial & occlusal surfaces.DrM*** discussed with patient a couple treatment plans to retain the teeth on the mandibular archTreatment plan #was to make a new maxillary denture and to place crowns on the remaining mandible to protect them, also by connecting the crowns would stabilize the entire arch and enable us to add a cantilever crown to #20, giving patient the advantage of not wearing another mandibular partialTreatment #was to simply fabricate patient a new maxillary full denture and madibular partial denture that clasped onto existing worn teethOn 8/18/patient decided to have treatment #Patient stated he would not spend that much money and did not want any crowns.October 21, we delivered maxillary full and lower partial denture.November 12, we adjusted upper denture & partial lower, patient left office very pleased.October 20, patient presented to our office complaining and wanted mandibular teeth built upHe was very loud and belligerent to staffDrM*** came in and explained to patient that he had that option up f*t & decided against itTo change the teeth now would affect the fit of the partial dentureDrM*** tightened partial clasps and it fit great.The following is the actual chart notes from 11/20/for you to review.Pt presents lower partial is not fitting too wellPt very unhappy and very loudPt did have two options when we proposed treatment plans, lower roundhouse vslower partialWhen DrM came in to room, pt started raising his voice & saying DrM needs to build up teeth because partial isn't fittingDrM tightened clasps, patient still popping it up with tongue, DrM explained he should avoid that because it will make it come upDrM explained this is what happens with lower partialsThey will get loose and we will need to tighten, pt started yelling saying DrM*** should build up teeth and DrM*** just took patients money and he's a crookDrM*** said patient was antagonistic from the start and dismissed patientPatient continued to yell, so no further treatment would be done and we dismissed patient from practice.In our office we do a great job accommodating our patient's needs and work hard on customer service.If ever we would have an issue with something that's outcome is less than expected, we graciously refund any moneys and refer patients to a specialist, etcI may have addressed this once or twice in my year careerIn this case we feel we performed services a year ago to the patient's requests, and therefore will not refund the patient*** *** acted very unstable in our office to staff and to myself, and therefore is not welcome to return.Sincerely,

[On Aug./ 4,at 1:PM - our initial appointment - according to Dr.M`s chart progress notes- was for comprehensive oral evaluation On Aug18,at 2:PM [our 2nd visit] we had an appointment for case presentation detailed and extensive treatment planning for upper dentures and lower plate. I don`t know what they are talking about when they say "already had exam elsewhere this year." We didn`t see anyone else for dentures. On Oct13,I called to make an appointment for *** to have his lower teeth built up so his plate would fit tightly. DrMhad told both of us that we would talk about what needed to be done with the lower teeth. It says in his progress notes that on Oct.20,at 11:AM we had an appointment with the description- case presentation detailed and extensive treatment planning. I made the appointment for this reason. We didn`t go there for an adjustment. DrMknew he wasn`t going to build up the teeth, since he put a stop on both sides of the plate t rest on my existing teeth. He needed an excuse for no treatment. He knew if he changed my existing teeth, the plate wouldn`t fit at all. We have the mold for the plate showing the stops on both sides.we would like to know your view on the matter.]
