Review: For past many years, I go to the same doctor for physical checkup. My insurance company covers 100% charges for preventative care/annual physical checkup. Past few years he has been recommending vitamin D tablets because mine comes little-bit below normal. This time I agreed to take higher dose than over the counter pills, so he wrote me a prescription for it. But now he is charging office visit as well. There were no extra test done or no followup visits were made. If the whole purpose of preventative care is to discuss blood-work results, recommendation, and other things, why I am being charged extra? Insurance company won't help me because they go by the code doctor has written in claim. Doctor's office says it was charged because of the vitamin D deficiency discovered during blood-work.Desired Settlement: I want the doctor's office and billing office to remove the office visit charge and re-send the claim to my insurance company with correct codes.

To Whom It May Concern:

We are responding to your letter of December 16, 2013 in concern to **. [redacted] complaint. We do understand “**. [redacted] concern” and we will try to explain our stand per medical rules. I have attached a copy of the page referring to this issue per the CPT of the American Medical Association. As you will see per the highlighted area the physician is allow to charge for an office visit of any E/M services performed on the same day of a physical. For this visit Dr. [redacted] did address the issue of “**, [redacted] vitamin D deficiency” and he did issue a prescription so this would warrant an E/M visit above and beyond the physical. Dr. [redacted] is a very thorough and detail physician.

We have discussed this particular issue and though we may not agree with **. [redacted], we will write off the balance that he owes and do a corrected claim to the insurance company deleting the office visit of [redacted]. We hope that this will help to resolve this issue that **. [redacted] has with his physician and the care that he receives from Dr. [redacted].
