Sent: Thursday, April 13, 1:PMSubject: ID NUMBER *** To whom it may concern, This is in reference to ID # ***, Dustin R ***. I believe his complaint is in regards to the laboratory's billed amount for services rendered. Patient was referred by Dr*** *** for a
urine full comprehensive Toxicology test, on 1-24-17. The services were rendered and reported to the doctor. We are an out of network laboratory that has a high fee schedule. We do submit charges to the patient's insurance company and request payment for services. Although we bill high, we do not get paid 100% of billed charges. We have attempted to get in network, for a lower fee schedule, but have been denied by ***. We do not balance bill the patient for charges denied by the insurance company. In Mr***'s case, the claim was submitted to *** for processing and the allowed amount of $was applied to an out of network deductible. This is the only amount the patient is responsible for. Also, it is the doctor's responsibility to inform the patient that he is using an out of network laboratory. We simply receive the sample and process, and bill the insurance for services rendered. I apologize that the patient's phone calls and messages have gone unanswered. We were experiencing issues with our phones. I have reached out to the patient today at 12:pm for a call back.
