This will act as our response to the complaint filed by [redacted] *** on 12-10- Our practice provided an estimate for services rendered with the understanding that after her insurance remitted payment, she could receive a statement from us for any unpaid balance on her account After working with both her medical and dental insurance carriers and receiving all monies they were going to render, our office sent [redacted] a statement indicating that there was still a remaining balance on the account The initial statement was sent out on 11-1-14, to which we never received any response from [redacted] *** On 12-1-14, our office sent her another statement with a "final notice" indicated on the statement, as the account had a balance that was approaching months outstanding On 12-9-14, [redacted] contacted our office to discuss her balance and explained that she would need to remit payment on 12-12-14, which we were agreeable to however, she also indicated that she would need to make installment payments on the balance as well It is our standard office policy that we cannot accept any payment arrangements within our office for any patient portion due on services rendered We accept the following forms of payment to try and assist our patients in getting treatment completed and pay any portion due by them: Cash, Check, Visa, MasterCard, American Express, Discover and CareCredit CareCredit also allows patients to make installment payments up to months with no interest charges provided the account is paid in full within months of the debt being charged All options were offered to [redacted] however, she declined to render any form of payment at that time and stated that she was not interested in applying for CareCredit either She then ended her call to our office abruptly On 12-11-14, we received a call from [redacted] ***'s referring dentist asking us to please make a special allowance for [redacted] ***, as she felt she was trustworthy and reliable to pay the balance off within the days she had requested We agreed to extend a payment option to her as a courtesy to the referring dentist, but could only extend this for days based on the current age of the account We were told by the referring dentist that after she discussed this option this with [redacted] ***, she was very appreciative and agreeable to the terms of payment to resolve her account with our office We were also told that [redacted] would be contacting us that same day to discuss the terms and dates to expect payment on the account To date, [redacted] has not contacted our office.On 12-11-and 12-16-our office left voice mail messages asking [redacted] to contact us so we could discuss the concerns she had initially contacted us about Because the messages were left on her voicemail, the specifics of why we needed her to contact us were not disclosed, only that we request that she return our call as soon as possible Our calls have not been returned We ARE trying to work with [redacted] by extending a day payment arrangement to her Considering the age of the debt, we feel we are being more than accommodating in trying to find a resolution that works for [redacted] *** We look forward to hearing from her to avoid any further concerns with her account Thank you
