out by client. Special prices were given for the Permanent Makeup —Brows regular [redacted] Eyeliner regular [redacted] The Special Prices can be paid by Check or Charge, & I
also offer a Cash Discount of [redacted] extra off each procedure given. I don't push any of my clients to do Permanent Makeup or to pay in Cash. All info, $'s paid are in my Client's Chart Notes. A dated receipt can & will be sent out to Client showing that $'s were paid by her.
am very thorough in measuring, picking colors,& matching Brow Designs to the Face shape SC Bone structure. This is all very important & done with each & every Client. I give all my clients the very best of my knowledge, education & experience, I'm Board Certified ( Must have accredited Education hours & Pass * Bloodborne Pathogens Class Yearly) also have 24 years of experience. I pride myself in my work & have many Happy satisfied clients. I'm known to be a perfectionist & very understanding & patient with all my clients.
A call was made to Client the next day after their procedure on Tuesday, September 29th, 2015 at €:05 pm. The Client said they were doing ok with some swelling on eye. They called 5 days later, Brows not even-higher. I reminded them that they need to let it heal for 30 days-nets days!! Another call was received on October eh, 2015 to tell me No Liner there &wanted a refund & they were not wanting to even do a touchup. Again I told them not to make a judgment as calor was still healing & curing in the
skin, I tried to explain to them that the color is there, but changes as the skin is cold or warm. They kept going on & stating how was color going to change & appear when nothing was there They weren't listening to my explanation of how the pigment heals. The Permanent Makeup will lighten 20-50% afterwards & 30% is average amount of lightening. Some people will be less & some more. I
encouraged them to come back in for the 1st touch up as it would make a difference. When they came back in for their touch Up on Saturday October 31, 2015 (I don't normally do touch Ups on Saturday, as I work Monday-Friday) I came in special for them. She had a 12:00 Appointment she brought her
husband back to my procedure room to talk with me about her Makeup. I asked him about her Eyeliner that she said Wasn't There! Did he see it, or what did he see? He said He did See it, but 1 side was lighter. S00000000-her telling me that nothing was there was not correctlill assured her I would make
it thicker. She showed me a picture of what she was wanting(which is good) 1 love photos as it helps me see exactly what the client wants. I told her I would go thicker with Eyeliner if she allowed me to work on her as she was extremely sensitive on herfirst visit. I started her Eyeliner At 1:18 & finished at 2:50. She wanted me to Join top & bottom Eyeliner on outside corners, I said Nol Not advisable to join top &
bottom together as that will make the eye appear smaller, not good if the client has small deepset eyes. I try to give my clients as much pigment as I can implant as long as they aren't too sensitive for me to work on them. It is safer if they lay still for both of us, Usually 2 visits (Appointments) are sufficient for

the application for Permanent Makeup-Brows-Eyeliner-Lips. But, on occasion I have a client that has resistant skin or skin that doesn't hold pigment. If that is the case I offer an extra visit for only [redacted] This $ is to pay for Probes used-Pigment used-Setup, No charge for my time. I use only the best Equipment & supplies. After I offered to do an extra visit, the Client wasn't interested in that, just a complete refund of [redacted] This $ was paid for 2V Eyeliner & 2V Brows & I gave her exactly that. I would he interested in seeing her Permanent Makeup now, but not doing any more work on her, est feet no matter what I did she wouldn't be happy. heel & believe we are both in agreement about me not applying anymore Permanent Makeup. I should have listened to my firsit impression & instinct of her, when she entered my business before her appointment. I introduced myself & told her ft would be a few minutes & I would be with her shortly; she Rudely said quite loudly, "Let me see your Credentials"
The Brows that were touched up an Saturday, October 315% 2015 were remeasured & drawn on & approved, as they were on her first visit on 9/XXXXX. The color was checked & was applied after the client's approval. Clients are given a Mirror (Regular & Magnifying) So there are No surprises when finished. They are also allowed to look as I work on them. I ask as I work, Is there anything they would like added or changed. I Never do any work till my Clients approve the Design & Colonl
do offer discounts for Cash, but always their choice. I never Push anyone to have Permanent makeup done. I have been in the business of Permanent Makeup for over 24 years. I offer my service & do my very best. I'm proud of my profession & my work. A Happy Client makes me Happy!
This Client making this complaint will not be happy with anything given & I feel they are being unreasonable by asking for a refund.
I'm sending a copy of my consent To Application of Permanent Makeup Procedure. Note Paragraphs 2-5 which were all initialed as they were read by Client, Signed & Dated. This to acknowledge that she read & understood everything she initialed. I'll also be sending Appoint Policy, read paragraph 9. I do have other Client Paperwork that can be sent if needed.

The response is untrue. She did not show me any photos of eyeliner she had done on the first visit She showed me photos of eyebrows and I stated I didn't like any of them. Hence the Molly design being chosen. I did initial the papers but as we were in a hurry I didn't read it as she told me it was a standard consent to do the tattooing. Yes, I did ask for credentials as I do with any dr. especially someone working on my eyes. She never told me it was a discount for cash. My right eyebrow was almost 1/2" higher than my left. Now I don't have the Molly eyebrows. I sent a photo of me without makeup - dark eyebrows and an extremely fine line of eyeliner. My husband did tell her that there was a trace of eyeliner in parts when I went for the 2nd visit but not a fine line as she stated. That is untrue. Obviously my skin takes to the ink - just look at the width and color of my ugly eyebrows. It doesn't matter how long she has been doing this - she was inept - no matter how she tries to explain it. I didn't expect her to admit she was at fault as she is an almost ** year old woman who is egotistical and a mutton dressed like lamb. My husband couldn't believe how much makeup she wore and her general appearance. Of course she is not going to admit she failed to give me anything close to what I wanted - it is not in her nature. I have called other tattooists and without seeing me but based on my info stated that in their opinion she did something wrong. I will have more photos taken this time by my husband so that you, her or a Judge can see what she did to my eyebrows and what she did not do to my eyeliner. I will provide these immediately if requested. She has tried to make this a personal vendetta by saying I rudely asked for credentials. I make sure every doctor anesthesiologist etc. are Board Certified - not just Board Eligible. In further substantiation to my claim I can provide information I researched on the Internet that will confirm my complaint. I thought the photos would be good enough to prove my complaint. Also, my eyebrows have never faded and as a result every time I color my hair I use a darker shade than I have for years so that my eyebrows do not dominate my face. Another statement that is untrue - she never offered to do a 3rd application for [redacted] - she told me in no uncertain terms that it was [redacted] My husband will confirm this as I told him immediately and he also thought it was outrageous as I had already paid [redacted] for something I did not get. He advised me to ask for my money back at the 2nd visit - she told me she could only give me [redacted] back as that was the 'wow-wee" touch-up cost. As the first time she assured me she would be able to give me what I had wanted just to be patient. I was patient and tried to be friendly while still apprehensive after her first attempt. By repairing the difference in height in my eyebrows - they are NOTHING like the Molly design. They are too thick and dark for my very pale complexion. The eyeliner did seem more what I wanted- however the bulk peeled in 3 days and the remainder is just an extremely fine line that doesn't do me any good whatsoever in applying my makeup. Eye shadow covers it up immediately. I should have realized that it would be virtually impossible for her to admit she did not give me what I paid for. It does not appear to be in her nature. Enlarged photos that I can send to you will just further substantiate my case. I was hoping to resolve this with the Revdex.com but she did not offer any money back and no I would not go back to her in a million years. It is almost like a cosmetic surgery gone completely wrong and the patient has to suffer from a misdirected hope and assurance for what he/she would get. I have to coat my eyebrows heavily in make-up which I have never done before so that people do not stare at these thick dark eyebrows. It takes a big person to admit that they made a mistake or that something went wrong and I don't think she has it in her.

Her response is repetitive in nature. States the same information as her previous response. Yes my husband did say he could see some eyeliner but there were areas that there wasn't any at all. In addition, what eyeliner was there was extremely thin - not at all what I had asked for. She did not show me photos of eyeliner just of eyebrows. I stated that I did not want the eyebrows of anyone on the tablet and the photos I saw.
We then picked out Molly design. The result after healing was virtually no eyeliner and my right eyebrow being almost 1/2" higher than my left. Also my eyebrows were not taupe but are a very dark brown. Too dark for a person who has very pale skin and born in England. I did make those phone calls and she did tell me to be patient. I did and after the period of time to heal I made another appointment. She worked on my eyebrows and lifted them with ink so the left eyebrow is almost as high as the left - this meant she had to do them thicker and no Molly design (I have forwarded a photo of what I wanted for my eyeliner and how my eyebrows look - I have also sent to the Revdex.com a photo of myself with no makeup to substantiate the fact I only have a paper thin evidence of eyeliner. Yes she did state she would not close the outside of the eyes and I was fine with that. She did go off the photo and it looked like it worked and although the eyebrows weren't as they were supposed to be with the swelling I could not tell the shape. I did comment again that they were too dark. She stated they would fade. This was the 'wow-wee' visit and she stated it makes all the difference.
On the first visit because there was a consultation first, she hurried me to start the procedure. I did sign the form she submitted but I didn't read it - she stated just sign the form - it is your consent for me to do the procedure. I admit that it was my fault that I did not read the form. However, when I have had other medical procedures done - they always give you a consent form to perform the agreed upon procedure and it is always the same. I thought this was the same thing.
I will submit another photo as I look now without makeup. I will email it to the Revdex.com.
She did not give me a copy of the form at the time of procedure. When I called her after the second procedure - she stated it would be
[redacted] for the procedure for materials - it is a complete lie that she said she would do it for
[redacted] I stated that twice she had not given me what I wanted and I wasn't going to pay another
[redacted] for something I had already paid for.
Again, yes I did ask to see her credentials. That is not rude but prudent.
Obviously the tattoo materials used do work on me as I have extremely dark eyebrows - not taupe as was chosen.
She has not given a reasonable response and it is basically the same as she gave last time.
You can forward the photo to her of how I look now without makeup and it speaks for itself. Dark eyebrows that have not faded - no Molly design and a paper thin eyeliner.
Obviously she is not taking responsibility for her failure to perform as she stated.
In addition, when she stated that she went in on a Saturday and she usually doesn't work on a Saturday - it was at her suggestion as she stated 'you probably want to get this done straightaway' to which I responded 'yes'. At no time did I ask for her to rearrange her schedule.
No I am not satisfied and will still continue to fight this as I have proof of what I asked for and also proof of what I have now. Neither of these are what I asked for.
As I stated again, she had another client who reported her to the Revdex.com because of eyeliner peeling immediately. However she was able to stop her check payment and that was how it was resolved. She asked specifically for me to pay in cash and did not mention anything about it being a cash discount. She stated on my initial phone call that if I had my eyebrows done with the eyeliner there would be a discount. At that time, I had not thought about eyebrows I was only interested in eyeliner. However, since my eyebrows were very light and quite sparse I decided that sounded like a good idea.
Again, I reiterate on the first visit and procedure - she DID NOT show me photos of clients with eyeliner. She did show me photos of eyebrows and I stated to her that I did not like any of them. The second time I made sure I had a photo of the eyebrows (with the Molly design) and eyeliner of what I wanted. She stated no problem. The problem is - I do not have what I want.
