Dear ** [redacted] :We received your letter on Monday, April 21,2014, regarding ** [redacted] ’ complaintWe would like to state our position by informing you that besides verbally letting all our patients know that even though we participate with their insurance company and submit their claims directly to them, we cannot guarantee payment or full payment from their health insurance companies, all our patients sign and date a certification of this [redacted] signed on 12/9/at 8:PM that “...I understand and agree that all services rendered to me will be charged to me, and I’m responsible for timely payment of such servicesI understand and agree that health/accident insurance policies are an arrangement between an insurance carrier and myselfI understand that fees for professional services will become immediately due upon suspension or termination of my care or treatment.” I have enclosed a copy to this agreement with her signature, please see attachment #1.It’s unfortunate that her particular health insurance policy did not cover certain charges incurred in our office, but which clearly the patient was made aware ofHer balance is, actually higher than that as of today, due to missed appointments that were schedule and patient did not call or contact us prior to hours, as according to the “No Call/No Show Policy”, which the patient signed on 12/10/(see attachment #2).If you have any further questions or require any other documents, please don’t hesitate to contact me at [redacted] .Best regards,

Review: Dr. Losagio was my chiropractor for 3 months. I received my first 2 adjustments and X-ray examination for free from a [redacted]. My insurance covered 30 visits a year, with a $20 copay each visit. I had scheduled appointments for 3 times a week - to correct my back pain. In the third month of my seeing Dr. Losagio, he said he had to take a second set of x-rays to ensure his plan was working. Since I wanted to be sure his treatments were working, I allowed him to take the X-rays. I paid my $20 co-pay that visit, assuming that my insurance had covered it, since there was no mention of it not being covered. Two weeks went by and I still had not seen my X-rays. I asked Dr. Losagio when he would show them to me, and he claimed he did not remember even taking them. He went into his office and searched through files and found my X-rays. He quickly pulled them out and compared them to my first set of X-rays, but did not explain what his treatment did or would continue to do, instead just told me that I needed to continue to see him to correct the problem because it was not fixed yet. He put them away and rushed me out the door because it was "closing" time. I decided to stop going to Dr. Losagio a few weeks after that. A month later, I received a detailed report of all my visits and payments, with a notice that I owed him $345 for the second set of X-rays he took 2 months prior. I just checked my claim through my insurance and it states that I was only responsible for the $20 co-pay (which I already paid) for that visit. I read reviews about the Losagio's office, and apparently this isn't the first time he's tried to wrongly charge his patients.Desired Settlement: Remove all outstanding charges on my account and do not contact me again.
