October 26, 2015Dear ** ***I am writing in regards to our conversation in reference to the patient copay complaintThe patient has insurance with *** *** *** *** *** ***, but she has had different id numbersFor policy *** with a coverage period of 8/1/- 10/31/
carried $copay per insuranceFor policy *** with a coverage period of 11/1/- 10/31/carried no copay.The patient’s husband, *** *** sent in a letter dated 8/21/asking for a $refund for copays that were paid for date of service 8/6/and 7/29/I called the insurance to verify what *** *** stated in the letterAt that time the insurance explained about the different policiesI called and left a message for the patient to call me so I could explain why they were only going to receive a $refundI never received a call backThe issue is not with Loudoun Medical Group/Family Medicine Associates of Alexandria, it’s with *** ***A Refund of $was processed 9/14/check ***.I have attached a copy of her eligibility and a copy of both claimsPlease feel free to contact me if any more information is needed.Thanks in advance for your assistance in resolving this matter.Sincerely,Ayanna G.Account Receivables

Dear [redacted]I am writing in regards to our conversation in reference to the patient copay complaint. The patient has insurance with [redacted], but she has had 2 different id numbers. For policy [redacted] with a coverage period of...

8/1/14 - 10/31/2014 carried $30.00 copay per insurance. For policy [redacted] with a coverage period of 11/1/14 - 10/31/15 carried no copay.The patient’s husband, [redacted] sent in a letter dated 8/21/15 asking for a $60.00 refund for copays that were paid for date of service 8/6/14 and 7/29/15. I called the insurance to verify what [redacted] stated in the letter. At that time the insurance explained about the different policies. I called and left a message for the patient to call me so I could explain why they were only going to receive a $30.00 refund. I never received a call back. The issue is not with Loudoun Medical Group/Family Medicine Associates of Alexandria, it’s with [redacted]. A Refund of $30.00 was processed 9/14/15 check [redacted].I have attached a copy of her eligibility and a copy of both claims.
