I purchased insurance through Louisiana Health Cooperative in Dec 2014 for a start date of Jan 2015. I am a 35 year old male with no health issues at all. I first tried to use my insurance in Nov only because of a car accident that I needed physical therapy. I was told that I was "termed" or terminated come to find out. Turns out they said I wasn't a US citizen even though I was born 70 miles from their corporate center. I sent them a copy of my passport, SS card, and drivers license when they asked me to show proof of citizenship. I sent it certified mail and never heard anything else. They cancelled my insurance without notifying me by mail or anything. I kept on making payments online and never knew I was cancelled. After finding out I was cancelled, I called LAHC and they said "Yeah we see your payments but our online system and our payment system are not linked together so you can still make payments online but it will not tell us you paid on a cancelled account". Really? Sounds fishy to me. Anyway they agreed to pay me back my 6 months worth of premiums, or $2600.00, by March of 2016. It is now April 19th, 2017 and I have not received anything in the mail or correspondence. I have also learned that LAHC was infiltrated by hackers and 8k members' info was taken. Never got an email about this either..kinda important don't you think. I also just read that premium refund are considered a Class 4 claim and Class 1-3 claims will be paid first and any money leftover will be paid to Class 4 claims. Really! Because I paid my bills on time and you told me you were going to refund me, I can't even dispute charges now that it is long way from too late. How convenient. ACA, LAHC and the Federal govt owe me $2600 plus interest and it looks as though I will get nothing more than likely. And they want us to buy Health Insurance through their exchange. Every time I call LAHC for an update, they can't tell you anything about when you will receive a refund nor can they tell whom to call. I really think this is absolutely ludicrous to not give me my premiums back for a service they were not providing.

Review: I originally applied for the insurance in December 2013 for myself and my daughter. In January I called to see when I could start going to the doctor and they still hadn't added her on the insurance. By March and then May of 2014, she still had not been added to the insurance. My daughter had suffered from severe headaches since she was 9 years old and it was important that she began seeing a doctor again. In addition to that while I was calling about her they continued to send the mail to the physical address instead of the post office box. I called them at least twice a week in reference to her and the address change and each person I talked to stated that they would change the address and add her to the insurance. I eventually had to go back into the market place and reapply for the insurance because they never made the changes. After they assured me she was covered we made an appointment to see a neurologist about one hour from our home. After we got there they informed me that she wasn't on the insurance even thou I had spoke to someone days before. She was eventually added.My daughter also spent one night in the hospital and even though I have insurance they are still sending me bills from collection agencies. they have also misplaced the payments. I have had to reschedule at least 8 appointments with different specialist and pay out of pocket for medications for my daughter because they informed me that they didn't have my payments even though I informed them of the dated it cleared my bank. On July 25 I had to fax them the cancelled checks showing the dated it clear my bank and theirs and they still have not cleared it up. As of last week the address still has not be updated.Desired Settlement: I would like for all of the hospital and doctor's bills to be paid including the deductibles and someone to confirm that this will not happen again.

Review: I purchased a health insurance policy for my wife and son through Louisiana Health Cooperative. I pay the premiums every month and this month was the first time I actually had to use the insurance. I brought my son to the doctor on 3/14/2015 and received a prescription for him. When I went to the pharmacy to pick up the prescription, the pharmacy informed me that the insurance came back as "inactive" as of 3/1/2015. My bank records show that the premium check was cashed on 3/5/2015 so they have definitely received payment for the policy (which has been the case every month). I had to pay almost $400 out of pocket for the medications, but I had no other choice since it was the weekend and the insurance company was unreachable. On 3/16/2015 I called Louisiana Health Cooperative to get this straightened out. I was told that there was some sort of "computer glitch", they confirmed that the premiums had all been paid, and I was told that the policy would be updated but the changes would take a day to go through and I would receive a call the following morning to confirm this. I received no call the next day. The pharmacy attempted to process the claim once again (over 24 hours after I was told that the error was fixed) and again, the policy is showing "inactive." There is a 7-day window for the pharmacy to re-file the claim, and tomorrow will make day 4. I am worried that Louisiana Health Cooperative will not remedy this issue in time and I will be stuck having to pay for medicine that should have been covered under the insurance. I always pay my premiums and Louisiana Health Cooperative is not holding up their end of the deal. My next step is filing a complaint with the state insurance commission.Desired Settlement: I need the insurance to cover this prescription as written in the policy. If Louisiana Health Cooperative cannot fix the issues on their end in a timely manner, I need them to reimburse me for the out-of-pocket expense. I would like to change my policy to another insurance company immediately, although I don't think this is an option. At any rate, I will not be buying another policy from Louisiana Health Cooperative.

Review: I pay premium on time with ID on check and it's not posted to account I then have to pay for my husband [redacted] diabetic Meds. They never return calls when you call they are on other line. This has gone on for many months I have sent copies of the checks from my bank to them and still problem precites. If I pay them on the first the check has made it through my bank by the 3ed and it's not posted to my account. This has been deposited to their JP Mogan account and yet it shows I have not paid. Therefore for my husbands health I have to pay for his Meds.Desired Settlement: They need to post to account has been paid.

Review: We have been paying our premiums on automatic bank draft never missed a payment but have been denied benefits and coverage for almost 6 weeks now. The exchange Obamacare says were in benefit but insurance company is taking our payments via bank draft and telling us were out of benefit and have denied our pharmacy claims and doctor visits. We are frustrated and have insurance but can't use it and going without medicines and doctors not seeing wife and children. Also have few thousand of in paid services. Review: I received a bill dated 2/10/15 with a previous balance of $477.78 and a current charge of $238.99 for a total of $716.77. When I called Member Services to discuss the incorrect billing, the representative could not explain the basis for the previous balance of $477.78 because she could see the payments in the system. However, she was unable to resolve the issue. When I asked to speak to a supervisor, the representative would not allow me to speak to a supervisor. She advised that she would send an email to someone in the company to escalate the issue and the person would contact me. It's been over a week and no one has contacted me. Every month there has been an issue with applying payment to my account since September 2014. Louisiana Health Cooperative incorrectly reported I had no insurance coverage for September thru December to the Internal Revenue Service despite receipt of payment each month. Each time I've called Louisiana Health Cooperative to resolve the issue, no one has helped. I don't understand where my payments are being applied. Louisiana Health Cooperative has not shown any interest in resolving this matter.Desired Settlement: Louisiana Health Cooperative need to correct my account by applying the payments I've made to my account. I have documentation to prove all payments have been made for my account. There is no previous months outstanding balance in the amount of $477.78. Louisiana Health Cooperative also need to correct the information sent to the Internal Revenue Service to reflect insurance coverage for all of 2014.

This company has been notified several times to cancel this policy, because I have received Social Security Disability. Review: Company now refusing to pay medical claims. The ones they did pay they paid months behind. I have called offices and had to beg for them not to send me to collections.Desired Settlement: Refund me all of my premiums paid, copayments,and deductible payments, or pay the claims.

Review: I signed myself and family of 6 up for a health insurance policy in April 2015 through the Marketplace. I paid the premium for the first month to LAHC at that time. On April 30, 2015, I cancelled the policy as we got a policy with a different company. On June 19, 2015, I still had not received a refund check so I called both the Marketplace and Louisiana Healthcare Cooperative and was told by both the Marketplace and LAHC that indeed the policy was cancelled effective May 1, 2015. The Marketplace told me I had to get the refund directly from LAHC. LAHC told me they would request the refund and it could take up 8 weeks. Again, the policy was showing cancelled May 1, 2015. I still have not received a refund. I have again contacted LAHC and now am being told it takes up to 90 days from the date of the request and nothing can be done until that time has passed. Once that time passes, they can then request a refund with upper management. As an individual, that is a great deal of money for them to hold until they decide to refund it. It has now been over 3 months since it was cancelled.Desired Settlement: I would like to be refunded what I paid in a timely manner. I believe 3 months has already been too long.

Review: I don't know if other people are having a problem with this ins. co. but I am. I was informed by my Dr.'s office that this ins. co. is not paying on my last Dr. visit to the amount of $530.00. I payed the deductible in full had to borrow the cash to pay it. the ins. co is to pay the rest not so? After some phone calls I still don't have an answer look co. up on web they are under court order? So I don't know what to do or about the bill at the Dr's office. only in Louisiana. Iam going to e-mail some friends in other states and see what they think of polatics as usesal in Louisiana. may send e-mail to the state ins comm. office and the att. gen. When you are on a fixed income and 60years old you don't need this BS They need to pay what is owed at my Dr's office and get the hell out of LA.Desired Settlement: PAY THE BILL IN FULL AT MY DR'S OFFICE

Review: I was accepted for a policy with Louisiana health cooperation on 12/1/2014. I pay my premium an entire month in advance.We picked a doctor off of their site that they recommend. one that their company said accepted this insurance in network.from day one I have had problem after problem. they terminated my insurance for non payment so I sent the check stubs in that went thru my bank. after I had to come out of pocket for meds they finally said it was a computer glitch and reinstated the insurance. I am yet to be reimbursed for the pharmacy. after 6 months of having this insurance I got a bill in the mail for 1218.00 for routine lab work. So I called the insurance they said that now my doctor is not in their network. keep in mind this is a doctor that I got from them. thats the only reason we picked this doctor is because they recommended it. they refuse to pay for anything at all because they keep saying its out of network. they only had two doctors in my area that accepted this insurance so now they only have one because they are saying she is not in their network anymore. I just wish they would pay what they are suppose to pay and stop giving me the run around. currently I am all paid up until aug 1st.they refuse to answer the phone and return calls. if I could afford it I would find another insurance but I cant until enrollment period. worst insurance company I have ever had. now im paying 400.33 a month for insurance that I cant use because I have to pay out of pocket for everything.Desired Settlement: I would just like for them to own up to their part of the insurance agreement. all lab work is suppose to be covered by this policy. they refuse to answer calls and return calls. please just pay the claims or give me a full refund for the policy. if I wasnt paying for this policy I could afford to pay out of pocket for the lab work. please answer the phone and get this straight. I just want my doctors visit to be covered like agreed apron.

Review: Premium was paid on 01/06/15 and again on 01/13/15 and when I went to the pharmacy to buy my medication, I was told that I was not covered even after seeing that the deadline for premiums had been extended to 02/28. This insurance company seems bogus to me.I have called on several occasions and been put on hold until the automated operator say leave your name and number and we'll call you back. Well good luck with that because YOU NEVER GET A CALL BACK. THAT'S PROBABLY WHY MY INSURANCE LAPSED.Also they charged me more than they should have. My premium was supposed to be $179 and shot up to $263 when Healthcare.gov said it should have stayed the same.If someone would just call me back and straighten this out instead of transferring me to someone else as soon as they can I would appreciate it.And you wonder why your enrollment fell short. Other insurance companies must be doing a better job.Desired Settlement: I would just like the problem resolved. Fix the price of my premium to what it is supposed to be and do what you say you're going to do.
