This complaint is from a Medicaid patient The State of Ohio has provided this patient a hearing device at no charge Our office provides assistance to qualifying patients with the required paperwork to receive a hearing device and then fits them with the deviceThe hearing device was sent back to a major manufacturer times for repair as defined belowThe first repair was sent to the manufacturer on 4/30/because the hearing aid button had been broken off and it was rattling insideThe receiver tubing had been pushed inside the aid dislodging the interior parts of the aidThis is a user problemThe aid was repaired by the manufacturer and we returned it to the patient at no chargeOn 6/9/the aid was sent again to the manufacturer for intermittent problems The aid was repaired by replacing the non-functioning partsThis is usually caused by a wax or moisture problem shutting the aid downAgain, there was no charge to the patient.On 6/27/the aid was sent in again due to patient complaint.The manufacturer did not find any problems with the aid when they ran diagnostic They replaced all the parts anyway.Again, the aid was returned at no charge to the patient.Recently,The patient called to report that the aid had again failed to operate properly.The patient claims that she mailed the aid to our office.She either failed to get a receipt, had not requested a receipt or lost the receiptThe aid was never delivered to our office Today, I called the manufacturer to review her concernsThey have agreed to replace the lost aid without a fee to the patient.We will provide the fitting services w/o fee to the patient and counsel her again on the care and use of the device-- Yours for Better Hearing, [redacted] Luebbe Hearing Services###-###-####

device was sent back to a major manufacturer 3 times for repair as defined below. The first repair was sent to the manufacturer on 4/30/14. because the hearing aid button had been broken off and it was rattling inside. The receiver tubing had been pushed inside the aid dislodging the interior parts of the aid. This is a user problem. The aid was repaired by the manufacturer and we returned it  to the patient at no charge. On 6/9/14 the aid was sent again to the manufacturer for intermittent problems.  The aid was repaired by replacing the non-functioning parts.
