My patient ** *** signed consent for fillings totally $ (the fees are different for each filling depending on size). The appointments were divided into separate visits, fillings totally $(teeth # 20, 21, 22) and filling totally $(tooth #29) due to the area they were
located. In addition to signing consent for services rendered, the Dental Assistant Melissa *** verbally reviewed treatment when seating the patient for fillings, and I also verbally reviewed treatment prior to giving the patient local anesthetic. The patient did not decline doing fillings. The patient never mentioned only wanting only filling doneThe patient was in the office for an hour for treatment. At certain points, I had to dismiss myself to do hygiene exams, and I let the patient know of the progress we were making. The patient knew that we were doing more then filling. At check out, patient *** complained of finances but did paid for the fillings. I had my receptionist call patient ** *** later that day and credited her account for of the fillings. So patient ** *** only paid for fillings totally $490. I also have other patients that only consent to filling at a time, and I do accommodate them. I can get a statement from them if needed.
