Review: I would like to file a complaint with the Revdex.com against [redacted] (plumber). Who we ask for him to install a backflow prevention device in April of 2011. After the installation he was supposed to submit a GEN-215B form to the Department of Buildings and he did not. When we called and spoke to him in 2011 (30 days within the installation period, when this document was to be submitted), he said that he had suffered a loss (his wife had past away) and he was at that time attending hr funeral, but that he would be submitting the paper work. Now three years later we are being held accountable for never submitting that form and are in the process of "failure to submit the initial test report as well as failure to submit the annual test report", which will result in the issuance of a Notice of Violation returnable to the Environmental Control Board with an imposition of a civil penalty of $1000 dollars. And may also result in an issuance of an Order to Cease and Desist and the termination of water service to the premises. When I tracked down the plumber, he was incredibly rude and malicious stating that he was never paid which is why he never submitted the paper work. However, the contract states that he would be payed in completion of his work, which he never did. By not submitting that GEN-215B form, he is in breach of contract and did not deserve payment at that time. He also made threats to put a lien on our property due to this lack of payment. But we sent him a check anyway with the form so he could fill out and send the form to the DEP. He still refused and said that he would not cash our last check and doesn't know if he'll fill out the form either. Now we have get a new plumber, new permit, perform initial and annual back prevention test. All before our last extension (by the DOB) which is April [redacted]. ot only that but his proposal clearly states that the balance would be paid after our final GEN215B test report is issued, so he is in breach of his own contract.Desired Settlement: I would like a full refund or I would like for him to submit the GEN 215B form signed, sealed and mailed ASAP to us so that we could meet the dead line and finish this matter. I don't know why he doesn't want to finish what he started. If nothing else. Then I want his license taken away for breach of contract.
