Review: After several issues with construction/quality issues (most of which the company refused to take responsibility for claiming that we can't prove issues of negligent workmanship) we have found a severe issue where we have essentially been breathing in mold for the past several years because of drywall left inside the duct system between the humidifier and the supply duct. Below is the email that we have sent the company. I have no means of attaching images to this Revdex.com complaint (at least in this field) but can pass along images that are referenced.

We're writing to all three of you as we are not sure who still works for Madison and who doesn't these days.
When we purchased our townhome (Lot #**) at the [redacted] in [redacted] in 2009 we had purchased the optional upgrade for the humidity systems installed on both HVAC units for the house. Over the past year, my husband and I have been having what we believed to be sinus-related issues for some time and have spent a good deal of time and money trying to see if there was any issue with the humidifier systems, since we had never been able to consistently regulate the humidity in the house.
We are expecting our first child next week and to be safe, by husband decided to purchase and install UV systems on the HVAC units to ensure that we have the cleanest air possible especially with having a newborn in the house. To our disgust and astonishment, when he opened up the humidifier cover on the HVAC on the 4th floor, he found a large piece of mold-infested drywall completely covering the left side of the supply duct (left side of the supply return as depicted in image1 attached) which appears to be the cutout from the drywall for the duct work into the wall (see image2) which was just left by your workers in the duct! As you can see from image3 attached to this email, this piece of drywall is covered in mold which explains why we've been having so many health issues as mold spores have been pushed through the ducts in our top two floor for the past several years (not to mention that the drywall covering that side would have resulted in major efficiency reduction of the airflow)!
We're hoping any remaining mold in the ducts gets disinfected by the UV systems that we've installed now, but this discovery is beyond appalling and only time will tell whether to impact to our health improves.
We're including the townhome's HOA board on this email as anyone else that may have had a system installed by you guys might be suffering from similar negligence as it's obvious that no one checked the installation.
We are at a loss about what to do as you guys have a track record of not caring to take responsibility for anything done in construction of the home, but at the very least we do plan on filing a complaint with the Revdex.com (even though you aren't a Revdex.com accredited company) to have a formal complaint on the record in the hopes that the health and safety of others are not compromised in the future.
RegardsDesired Settlement: Not sure what would be acceptable/adequate. Would like Madison to stop being so negligent and if they are able to provide any compensation (for the many repairs, the UV system we had to install, and medication/doctors visits that would be good, but it would be hard for us to tally all of these expenses over the years) ... main concern is to hold them accountable to health and safety as our experience with them has been painful and we've had to do a lot of repairs on a home that is now only 4 years old (note that most of our back and forth was within the first year while they were responsible during the warranty period but we could only get them to do a few things, for example they didn't think it was their responsibility or flaw in construction to fix roof tiles that blew off of our roof in the second year of the home among many other issues!)
