Renault came in for his initial exam on 11/15/ At his consultation, he decided he wanted to do part of his work in 2014, and complete it in to maximize his insurance benefits?He signed his consents, including the sedation consent that states he may be unable to discuss treatment options and that if a change in treatment is required that he authorized the dr to change the treatment I have attached the signed consent.Renault came in for his treatment on 12/5/ He stated to [redacted] that he takes [redacted] every night and that what we gave him to take prior to coming in hadn't had any effect on him [redacted] examined the patient and found that he needed additional work that was not addressed at his initial consultation The patient agreed to have the work done Additional sedation drugs were administered, and treatment was started.We strive for optimum patient care as well as patient satisfaction [redacted] has tried to contact the patient several times to discuss this complaint and to try to reach an agreement

** *** communicated with ** *** and his fiancee earlier this week and agreed to refund ** *** $ ** *** signed a release of liability and his fiance picked up the refund check 1/28/ We believe this should satisfy his complaint about necessary treatment being performed without prior authorization

I believe *** *** is misconstruing the factual events of the day The dental procedure I completed in ** ***s office was scheduled 12/5/ I went in his office and paid for my procedure in full on 12/2/ During this visit I was given several prescriptions, one in particular had to be taken min before my appointment in 12/5/that would start the sedation process I had taken the prescription as prescribed, this medicine advised me against driving and I had to have my girlfriend drop me off due to the side effects of the medicine making me drowsyI proceeded to the dentist office at me scheduled appointment time I sat in the dentist chair approximately minutes before ** *** entered the office, he asked me about my current condition and I advised him of being slightly drowsy, but not completely under yet ** *** then crushed up two *** pills, and had me swallow the medicine and left me again to sit in the chair min later he came back to start the workAt this time I was completely under the influence of the medicine, I recall him working in my mouth for quite some time then Informing me of some additional work I would possibly need I did not and could not have authorized any additional work, due to being incoherent I do recall asking for someone to call my girlfriend and I am unsure of when she arrived The signature ** *** is referring to is for the scheduled work I paid for, nothing additional I have attached the payment in full for the work I authorized that day I also have the receipts and estimates for the work I needed The additional work ** *** performed was absent from the estimate The manner in which ** *** performed the additional work was unethical and against my permission The *** the that ** *** mentioned is also incorrect about the dosage I informed his staff of the current prescription I have for the *** in which I take one per night to assist with sleep This is why I declined the prescription for *** his staff suggested I also find it interesting that ** *** said he attempted several times to contact me and I donot have one call or voicemail, yet I have left him a message I would like to realize this matter before I have to resort to legal action and formal complaints to the *** dental board and news agencies I would like to receive a complete refund for the additional work that I didn't approve of, and resolve this matter before I have to resort to the above mentioned steps and more*** ***
