As of Friday, September 26, 2014 at 11:31 AM, this issue has been resolved to the satisfaction of Mr. [redacted] (see email string at bottom).
This complaint was mistakenly filed against Avenue 25 Advertising and Design, LLC., as Avenue 25 has neither done business with nor processed a credit card...

owned by [redacted] or [redacted]. The correct business he is complaining about is a trucking industry website [redacted], a division of LoadPilot.com which is a subsidiary of [redacted]. ([redacted]). [redacted] is a completely separate business from Avenue 25, however both businesses are owned by the same individuals and are managed out of the same offices and we assume this is where the confusion stemmed. Regardless of this error, [redacted] has resolved Mr. [redacted] issue with a full refund of $10. The initial account charge of $5 was mailed to him in the form of a check and the second month’s $5 was refunded directly to his credit card.
After considerable research, Dynamic Application Services discovered that Mr. [redacted] credit card had indeed been used to set up an account on [redacted] under the name of [redacted], using the business name [redacted] and the email address [redacted]. According to the [redacted] website, that company’s contact information is [redacted], Phone: ###-###-####.
[redacted] was unable to find information regarding a company operating under the name KC Transport Solutions, LLC., and an email address [email protected] that Mr. [redacted] had provided to us in a phone call was non-functional.
Here is the most recent email string with the customer relating to this topic:

The email you provided me [email protected] does not exist.  I am using the email that the Revdex.com provided.  Please read below for the 3 emails sent to you regarding your account set up by [redacted]
I did some more digging and looked by your last name based on your Revdex.com complaint.  I came up with this account:  This is not the same email you gave me, but this person has opened an account with your credit card information in the name of [redacted].
[redacted] of [redacted]
Customer Code
Email  [redacted]
I show that your account was charged on 7/21, 8/21 and then was cancelled so a total of $10 had been charged but no further charges have incurred and the account was cancelled when you requested.  The system will only allow a $5 refund due to the length of time.  If you provide me with a valid name and address, I can have a check issued for the remainder.  Please confirm you received this email.
