We advised all our patients during their initial intake process that our services are provided with a minimum fee
(which is what the complainant paid) AND with the remainder of the usual and customary charges based on a sliding fee scale based on income, family size and federal poverty guidelinesWe also advise the patient they will be placed at 100% (full) pay until we have proof of family size and incomeMs*** has not provided information sufficient to place her on sliding fee scale therefore, she is being billed at 100% of usual and customary chargesIf she will provide needed information, we will "slide" (adjust)her charges accordingly, if she meets criteriaWe require proof of income (federal tax return, or food stamp verification, or social security info, or check stub, etc.), and proof of residency (utility bill)Providers have been advised to refer all questions regarding billing/collections/write offs, etcto appropriate Business Office personnel so as not to give out the incorrect information

I was never dislosed that I would be charged more than what was requested at time of serviceI asked the Drspecifically how much it was so I could make a decision as to whether I could afford to do any more workNever and I say again, never was I given any indication nor can they show that I was ever disclosed that I would be charged more than requested at time of serviceI have the right to determine how and where I choose to spend my hard earned moneyI was never given that optionI would not have taken off work seven different days and driven two hours round trip because the Drsaid he could only work on two teeth at a time because of the type of service they are to pay full feeIf, and if, although I would not have chosen to spend that much money on my teeth, at this time, but if I had chosen to, I would have gotten the work done in one day not seven days and saved losing pay from workIt,s utterly ridiculous that anyone would believe you could only do minimal work each time and still be charged a full feeThese people took my information and without my permission or, even the option to decide, have run up a bill without my knowledgeThis is stealing, against any ethical or professional way of doing business and ultimately deception in the manner they have chargedI was never given knowledge of the charges and have the receipts to prove there was no running balance which shows evidence of their deceitAny other business would have a running balanceWe all deserve to know the charge before the work is done, not months later!

Revdex.com letter not received in our office until 8/19/Account will be written off in its entirety to settle this disputeAs a matter of point, Complainant reported directly to me as my Executive Assistant, so she knew the rules!!!

If I understand the response from "*** *** correct, they are agreeing to to internally eliminate the alleged charges or billingThis will not reflect on me in any wayIf this is the agreement, I acceptI ethically should not have been billed an unknown charge to begin withCustomers should always have the right to decide to accept or reject services based on price
In reference to the addition comment, I am not sure what was meant by "the rules", nor do I know who is writingObviously, I do not know the internal "rules" of their business, but as explained above, rules of ethics are disclosure of pricing to your customers
I looked up the board members on the internet and attempted to communicate with *** with no avail
To conclude, if we have come to this agreement I appreciate the professional integrity to correct the misunderstandingThank you
