you referred to as (freebies) was paid at 100% by your insurance.  As of today, you have paid us nothing out of pocket, as you were supposed to per your insurance contract for all the fine treatment we provided you.  We enclosed the explanation of benefits (we received from your insurance company) the same as you received that clarifies the facts.  You did, in fact, authorize all treatment we performed and we enclosed fee authorization with your signature.  Of course, you have not responded to our statements for over a year and now it has gone to the collection attorney.Be advised, although attorneys are expensive for both parties, should it be necessary, I will aggressively defend my reputation.
