The worst experience I have ever had with a doctorFirst let me start off by saying that I am not the most easy going patientI ask alot of questions and I need to know extra information, I have ADHD and those are my little quirksEvery other doctor Ive ever had has been understanding and accommodating everyone till nowDrK and his staff at Rockville MOSA treated me with the utmost disdain and annoyanceAnd I am NOT that annoying of a patientLet me explainFor my first visit I came prepared with my insurance information, fee schedule and referral from the doctorI knew my out of pocket costs for a consult and panoramic xray should be 0$I called to confirm with my HMO Dental insurance D*** D*** I have the 710xs FEDVIP plan which is a "discount plan" and according to my fee schedule Consults are $and Panoramic Xrays are $as wellSo imagine my surprise when I showed up for my Sept 12th consult the front desk rep ,Ryan said ok so its going to be $153." What? No, that's not right according to my fee schedule it should be zero"Ma'am we double checked it and that's your cost, can you do $90?" "NO! Im not paying anything cause its covered by my FEDVIP 710sx plan" I leave distraught and a wasted a day off of workI call my insurance and confirm that INDEED my out of pocket cost should be $0.00! I call them back to reschedule and confirm and reluctantly they agree but now I have to bring in a Maryland Uniform Referral FormOk Perfect!
So one week later I show up with my referral form and just my luck my referring dentist forgot to put down my insurance information in the upper right hand cornerWhich is information they already had but were adamant that the referring Dentist HAD to put it in that box themselves(Im rolling my eyes and getting angry at this point because cmon who is gonna know that someone else filled in that information) WHATEVERI call my dentist who is block down the street and ask for it to be faxed overIn the mean time I ask if I can proceed with my appointment while its being faxed overThe receptionist lady behind the counter is very rude and unhelpful and adamant "NoWe can see you till that is filled in" So I LOST ITI had a breakdown RIGHT IN THE MIDDLE OF THEIR OFFICEI cried and balled my eyes out and caused a slight scene "How can you guys do this to me? Its on its wayWhy cant I be seen in the meantime?" "I cant believe I took another day off of work and blah blah blah" Yea I acted badlyBut I had had itI was frustrated beyond measureNot about the form but their treatment of meThey acted as if I was trying to steal services from themThey acted like I was some low life scumWere VERY unaccommodatingFinally the Office Manager patronizes me and calms me down and DrK sees meMy case is complicated because I need a coronectomy on the right lower wisdom toothI had to get a CT scan and the Dr K confirmed the prognosisSo I get to finally schedule my appointment and I request nitrous oxide and ask if its ok with the Drand they confirm its okWeek later I call one last time to double check what I would need to payI call my insurance to confirm an estimate of what I should be paying, they offer to call the Dr to double check and I think that's what set off this whole fiascoSo Dr K's staff tell me itll take a day to gather the estimateI wait all day for a callI call back today this morning and they tell me "we are still working on it" meanwhile the receptionist gives me some ADA Codes that don't quite match up to what surgery Im having done but that's OK cause Im not doctor! Im thinking why cant I just get a quote like that? Finallly Dr K calls me back and he DUMPS MEHe says his staff is bothered by me and I make too many demands and he doesn't feel comfortable doing my procedureSays I should find some one elseWHAT?
Because Im asking for an estimate and double checking information on my procedure? Something is rotten in DenmarkI come on yelp and I noticed theres a slew of hidden reviews all stating that they were being overchargedSo Im definitely filing a report with the Maryland State Board of Health, Dominion Dental Insurance Fraud Investigation Unit,State Attorney General anyone else I can think ofIf you had a similar situation I suggest you do the sameAnd check out the hidden reviewsIDK how they got them hidden but if I had seen those ahead of time I would have picked someone else

I am rejecting this response because:
Some of the statements made by MOSA are incorrect or half-truthsI am going to correct them and attach some evidence to back up my rebuttal.
"...her second discount plan was not yet in effect." - I was erroneously dropped from the 710xs *** Dental Plan which allowed the *** Plan to become primaryOn my first visit, I confirmed with "B***" (Name has been changed) that he was quoting the $190(?) from the 710xs--right? And he shook his head yes, and kept repeating that it was a "discount plan"I kept saying "but the 710xs plan says $for consult and $for panoramic"To which he replied "Your paper must be out of date or incorrect." Had Mr*** double checked the Plan id he was looking at he would have noticed that in fact he was not looking at the 710xs planHe should have called the insurance company to reconfirm, which is what I did right after I left the officeAnd the insurance company informed me that I was erroneously dropped and they would put me back on the 710xs plan immediately and I could still be seen that very same day But by the time I got off the phone with them it was too late to try to reconcile my missed appointment"*** *** Presented a blank referral form to our staff" Thats gross misinformationMy dentist filled out the MURF form (including signature) but left the "carrier" aka insurance information blankSee attachment 2 I questioned the immediacy of that information: "Can you just put in that information yourself? Its the same insurance company! Are they really gonna know someone else filled out that part?" NopeSo I urgently got ahold of my dentist so that she can fax over the filled in part"Can I be seen in the mean time, while she faxes it over?" To which the chubby lady behind the counter said nope As you can see the rest of the form is filled out, including my dentist's signature which MOSA claims "it required doctor's signature"
"She continued to argue at this point, shouting, becoming verbally abusive to our staff using profanities" The most abusive language it ever got to was me saying "I cant believe this st is happening to me again" in reference to my first wasted day off of work which could have been avoidedYea I got a little loudI NEVER made any threats to any staff or intimidated anyoneHad that been the case MOSA should have phoned the policeInstead they calmed me down and I was eventually seen that very dayDoesnt sound like the actions of staff who felt "threatened or verbally abused"
"The referral would be obtained afterwards" False! I was seen only after the form was confirmed as faxed over.
"Just prior to her appointment, however, she informed our office that she was changing from IV to nitrous oxide" Not trueI called and asked the receptionist if it would be ok to switch to Nitrous Oxide as it was cheaper on my planI asked her to double check with the doctorShe said OK"These issues were all resolved prior to her initial visit" Not trueI had to call several times to get a quote of the exact price my surgery would beI was not given any estimates which seemed odd to meWhen I called expecting to get a quote, the receptionist told me she would call back and thats when Dr K*** called me back and "dumped" me

My daughter needed all of her wisdom teeth removedShe went to see Dr.Francioni and she said my daughter needed a CT scan because her xrays showed one particular tooth was close to the nerveWe had the scan done and dropped the results in the office and they said they will call to schedule the surgeryA week later we hadn't heard anything and my daughter was in a lot of pain this whole timeI called the office and the receptionist was very rude and told me the results were on her desk and she didn't know when the doctor would be looking at themI told her my daughter was in a lot of pain was there a way another doctor could look at it and get these teeth pulled She told me no and that she doesn't know what I want her to tell meThis women was very insensitive and very rudeWe never heard from the doctor or anythingI will never go back and will never recommend this office

I had a very stressful experience with a receptionist earlier this year over an insurance issue. She was extremely unpleasant and unhelpful. Fortunately once I escalated the matter to the Office Manager, it was resolved very professionally.

Review: Let this letter serve as my official notice of grievance with Maryland Oral Surgery Associates. Specifically towards Dr. K[redacted] and his staff at Maryland Oral Associates in the [redacted] Campus. Please note that a copy of this complaint has been sent to the Federal Trade Commission, Revdex.com, [redacted] Dental Insurance Compliance Department, [redacted]! Inc and The Maryland Insurance Administration.

On August 29, 2014, my general dentist Dr. Lili G[redacted] of [redacted] in [redacted], MD examined my wisdom teeth and advised extraction of tooth # 17 and #32. That day I looked at my provider list through my FEDVIP [redacted] Dental Insurance (Attachment B) plan. The first Oral Surgeon that popped up with Dr. Steven K[redacted] at Maryland Oral Surgery Associates (Attachment A). I called that week to make an appointment for a consult. I gave the receptionist my name, contact info and insurance information. My appointment was set for September 12.

Because I am an informed patient (colloquially dubbed Petits Papiers among medical professionals) prior to my appointment I made sure to confer with my 710xs Fee Schedule. (Attachment 1) Per my fee schedule I would expect to pay $0.00 for the Panoramic X-ray and $0.00 for the Consultation Fee.

Imagine my surprise when the African American gentleman, [redacted] (or B[redacted]) try to collect a $153 payment. I referred him back to my fee schedule and confirmed my 710xs FEDVIP plan. He was adamant and firm that my fee schedule was "outdated" or "changed" or "wrong" when in fact, I literally JUST received that fee schedule with my Insurance card in the mail one week prior. Since I did not have the fee schedule with me on hand, I could not produce the proof immediately. Once again I insisted that I should not have any out of pocket costs. [redacted] countered with, an offer to charge me only $90 and I pay the rest later. No dice. I did not have enough funds to cover that expense as I was not planning on paying anything.

I left distraught and confused. I immediately called [redacted] Dental and they indeed confirmed that Dr. K[redacted] and MOSA were approved providers and per my fee schedule I should be paying $0 out of pocket.

That did not matter for the Associates at Maryland Oral Surgery who fought tooth and nail with me the next day when I called in, insisting that my insurance is a "Discount plan" and that this is what they charge people who have that kind of plan. I called my insurance company to complain who said they would give them a call. The insurance company representative came back on the line frustrated as well. "Im sorry ma'am I could not get them to accept to the terms of their provider agreement with us. I will have to further escalate this matter."

So the next day I call the office manager of MOSA to get things straightened out once and for all. She says she will contact the [redacted] office. Once she got things settled they finally agreed to the $0 fees for the pan x-ray and consult with the stipulation that I have my referring doctor fill out a Maryland Uniform Referral Form (Attachment 2). I made my second appointment for Sept 26.

September 26. I arrive for my consult appointment with my forms and my "little papers" to make sure I am not being overcharged. One of the chubby billing specialists (I don’t know her name) informs me that my form is not filled out correctly and I cant be seen till it’s completely filled out. (Attachment 2)

The missing information in question is the insurance information ([redacted] dental). Even though she could have just filled in that information without anyone knowing, I understand they are trying to do things the right way and this was part of their policy. However, if this was the case, why wasn’t I told to fill out a Maryland Uniform Referral form when I first made my appointment? The only form my referring dentist was given to fill out was some MOSA online referral form.

Per the chubby billing specialist's request I contact my general dentist to fax over a corrected form. In the meantime I inquire if I can proceed with my appointment while waiting for the form. Very callously and adamantly the chubby billing rep says "No, we can’t see you without the form." I make a final plea to get the ball rolling because I have another appointment scheduled with my dentist. She hangs her head low and shakes it to say No.

So there in the middle of the office I lost it. I had a breakdown. Am I proud of my moment in distress? Yes and No. By melting down in the middle of the lobby of the MOSA [redacted] Office I let off a steam of frustration in dealing with unapologetic staff

who treated me like I was annoying or I was trying to get something for free. All I was attempting to do was be seen and not pay anything I wasn’t supposed to.

Finally the office manager brings me into her office and calms me down. I don’t know if she was genuine or if she was patronizing me. Probably the latter but she definitely acted in a compassionate manner. I appreciate her for that gesture, genuine or not. After that fiasco I am finally seen by Dr. K[redacted].

My pan x-rays are inconclusive and he needs a CT scan to verify where the nerve is in relation to my wisdom teeth. He writes a prescription for CT scan at Shady Grove radiology and I mention that because I have [redacted] I don’t think I can go to the recommended facility. I get the CT scan done at [redacted] that very same day. Monday I drop the CD off to the MOSA [redacted] office.

CT scan is fuzzy so Dr. K[redacted] has to tweak it with some software in his other office. After 1 week of consulting with my dentist and orthodontist (although I dont think he ever contacted Dr David Lavine of Maryland Advanced Orthodontics) he recommends a coronectomy for my lower right wisdom tooth and a full extraction for the lower left. Finally I make my appointment for surgery October 30th at the Bethesda campus where Dr K[redacted] will be that day.

Dr. K[redacted] claims I was making many demands that made his staff feel uncomfortable.

Call Inquiry Synopsis

Here is a synopsis of the calls I made:

I make several calls to the office in preparation for my surgery.

Call 1 - Doctors note for my job so I can have 2 days off. I also inquire about surgery instructions as I was not given any information with regards to prep (no eating, attire, jewelry, someone to accompany me etc)

Call 2 - Inquire about nitrous oxide. Spoke to the receptionist and specifically asked to make sure it was ok with Dr. K[redacted] if I use Nitrous Oxide instead of deep sedation as it is cheaper. Because I have two plans with [redacted] dental one through FEDVIP and one through Kaiser. I double check about using the nitrous oxide through the [redacted] plan with [redacted] Dental and they confirm that yes it can be done. I relay this to MOSA and the receptionist says OK.

Call 3 – Call to the Bethesda office to confirm out of pocket costs. The receptionist says she needs to get back to me and will call within a day. I wait all day. No call. I call the next day. While talking to the receptionist I mention that I am taking Adderall and ask if what local numbing agents the Dr will use. She says Lidocain and Markcain I then ask if it has epinephrine in it and ig it’s contraindicated that I should quit taking the adderall for the day because I may have a reaction to the epinephrine in the Lidocain and

xylocaine (Prior experience with Local Numbing agents gives me a racing heart and that reaction combined with Adderall is dangerous). She said she doesn’t think so, to which I politely (not demandingly) explain if she could double check with Dr K because I have had a reaction in the past. I should ask Dr. K and that he was in the [redacted] office today. I didnt even get to call back because Dr. K[redacted] called me back to fire me as his patient.

Dr K[redacted] No Longer Wishes to Treat Me

Here are the reasons Dr. K gave for dismissing me as his patient;

• I made too many demands, (staff “perceived” me as being demanding)

o That’s incorrect when I spoke to staff members on the phone every request I made was a QUESTION or an Inquiry not a demand.

• His staff feels uncomfortable

o This is not my concern for I never made any threats or physical intimidation on any of the MOSA staff. As a person with ADHD I had an emotional meltdown because I had not taken my second dose of meds and I was dealing with the stress of an uncaring staff. As medical professionals I would expect the staff to be understanding. If they felt uncomfortable then why did they not ask me to be dismissed at that point? Why wait till 1 week prior to my appointment to voice their concerns? Why did Dr. K[redacted] wait three weeks of evaluating my CT scan, giving me a treatment plan and setting up an appointment?

• I Keep Calling

o Yes, I did. I have called MOSA at least 10 times. To straighten out financial matters, to ask about IV sedation vs Nitrous Oxide, to sort out insurance fee schedules, Ive talked to the administration business dept when the MOSA [redacted] office would not accept the terms of the provider agreement, I called about my CT scans, I called to find out when I could schedule my surgery appointment…etc Yea, I called a lot. So what? Where is it in MOSA policy that a patient make multiple calls about their treatment?

o If I was given proper info I would not have to keep calling. I was not given pre surgery instructions, my billing information was grossly incorrect and I had a complicated treatment plan.

Conclusion

I find it very suspicious that after I called to inquire about my out of pocket costs that all of a sudden Dr. K wants to “dismiss” me as a patient. The MOSA staff member quoted me the following on call #3 as the codes for the procedures I would have done.

Procedure MOSA Bethesda gave me these ADA codes

#32 Coronectomy 7240

#17 Impacted WisdomTooth that’s partially exposed 7230

Infection Control Fee $27 (A fee I later found out is a violation of the HMO contract with [redacted] Dental)

Compare to Attachment 1 codes

I suspect that MOSA is overcharging patients to recoup costs not covered under their HMO capitation agreement. (Attachment 3) is a copy of the agreement between [redacted] dental and MOSA. (Attachment 4) shows [redacted] Reviews that have been mysteriously hidden leaving behind only 2 suspicious 3 and 4 star reviews.

Remedy

I don’t want anything from MOSA or Dr. K[redacted]. You can’t compensate me or apologize to me for my wasted days off of work or my frustration or even my tears. I just want an investigation on the finances and insurance billing procedures of this establishment. I have interviewed a few other patients who have had similar experiences in being overcharged fees not in their plans fee schedules and I think there is cause to look further into this matter.Desired Settlement: I don’t want anything from MOSA or Dr. K[redacted]. You can’t compensate me or apologize to me for my wasted days off of work or my frustration or even my tears. I just want an investigation on the finances and insurance billing procedures of this establishment. I have interviewed a few other patients who have had similar experiences in being overcharged fees not in their plans fee schedules and I think there is cause to look further into this matter.

After reviewing its billing practices, MOSA should set up a universal way to inform patients about out of pocket costs. After going to a new Oral Surgeon and seeing their process and how smooth it was I believe MOSA could benefit form the following process.

1. Patient Calls MOSA for a consult. > Receptionist Verifies Patient Insurance, records contact info and sets up appointment (this is a good opportunity to let the patient know how much their out of pocket costs are for a consult) > Receptionist informs patient about Maryland Universal Referral form.

2. Patient comes in for appointment > Fills out new patient forms > Billing specialists looks up fee schedule for consult and INFORMS PATIENT ABOUT COSTS using clear concise information. > Patient is seen ,if doc OKs appointment for surgery go on to the next step.

3. Patient visits with billing specialist and they go over the costs line item by line item. Explaining the ADA Codes and out of pocket costs.

And so on and so on...

I am rejecting this response because:

Review: My wisdom tooth on my lower right jaw was swollen so I went to see my dentist who said I had to go to an Oral Surgeon as the tooth was close to a nerve. She gave me a referral to Maryland Oral Surgery Associates. I made an appointment for 04/29/13. I was assigned to Dr. [redacted], who assured me that he pulls out these teeth in 5-10 minutes. He did the procedure. In the process, my nerve got damaged in the process. I went to see him for a follow-up consultation to discuss the headache, as well as the burning, pain, and tingling sensation in my jaw on 05/03/13 at 2:00pm. Dr. [redacted] told me it was TEMPORARY PARASTHESIA and was assured that it would resolve in two weeks. When it did not resolve I had another follow up consultation with him on 05/13/13. He gave me yet another diagnosis of HYPERSTHESIA, and put me on a regimen of [redacted] Dose Pack. Even though the medication reduced my pain, it always made me feel weak, and I realized I was bruising very easily (have not had bruising problems since I stopped taking the medication). However, as soon as I finished taking the medication all the symptoms came back. I went and saw Dr. [redacted] again (05/31/13 at 9:45am) who suggested that all he can do for me at that point is to prescribe the same medication ([redacted] Dose Pack) for me because as there is nothing else he can do for me. He also gave me a third diagnosis of Neuroma, because as he explained to me, scar tissue may have formed on nerves. He recommended that I go see a Micro neurosurgeon, and that he will be sending me a referral. He also promised to refund my out-of-pocket payment ($312.00) I made at his office. After several phone calls, he gave me a referral to a Micro neurosurgeon whose practice does not exist anymore, and another one to a neurologist within my insurance network. He however told me he will not be able to give me the whole refund as promised previously, but will give me 50% by check. I never heard or received anything from him for several weeks. When I called to ask about it, I was fortunate enough to reach him this time. He apologized and said he had given the go-ahead to his front desk staff, and that he will remind them again. If I did not receive the check after 1 week, I should call back. I did not receive the check, and after several calls and messages left at the front desk, I was told by one of the front desk staff (on 08/12/13) that no such refund has been authorized by Dr. [redacted], and it is probably something he was doing personally. I left another message for him to give me a call back, but I have still not heard from him. Meanwhile, I still have pain in my mouth, and have to numb my gum on the right lower side before I floss. Meanwhile I have had a consultation with a neurologist who told me to give it approximately 1 year to see if the nerve will heal.

I just want Dr. [redacted] to honor his promise of giving me 50% of what I paid. After all it is the very least he should do after damaging my nerve resulting in constant sensitivity and pain on my lower right jaw. A condition nobody can guarantee will ever improve, even with time.Desired Settlement: Refund what he promised to refund. A total of $156.00.
