way to initiate a claim. Per her request, it was mailed to the address she confirmed on Friday 3/31/17. This is the only time I am aware she had called requesting a claim form. Though we strive to offer the best service with packing, loading, delivery and re-assembly of furniture sometimes there is damage. We have never refused/denied a request to file a claim, in fact Ms. [redacted] filed a delay claim (which is absolutely her right to do so) and was settled last week. I followed up with Ms. [redacted] and e-mailed the claim form.
