has not been used so it would total out to $I am going to call him and try to speak with him and let him know that I understand his frustration but sometimes therapist get sick but we do try to reschedule the appointment within the next day or so

different location, I cannot see his whole file and cannot make any kind of arrangements with him. All of this has to be done through his home clinic.

We have tried to reach out to [redacted], via phone call, multiple of times within the last 30 days and have left several messages for him. We have agreed to cancel out his membership without him owing anything else on his account. With that being said his membership is completely cancelled out and all sessions on that account have been cancelled out with it.

transfers to the Southlands location. Per our records we have no invoices for the requested dates to provide the member with. We have attempted to contact the member to advise her of this and the number we have on file is not a current number.
