Dear Revdex.com and *** ***,: I have reviewed the response made by the business in reference to complaint ID ***I would highly appreciate if *** ***can send me an official letter to clear my financial responsibility of the bill, which incurred on 02/**/for my blood test. To clarify, I wish that *** *** can understand that 1) I do not need to get a prior approval from my insurance for blood work performed in the facility of an in-network provider as long as I have a prescription.2) John TMather Memorial Hospital is listed in the provider directory for my insurance plans, including the blood workRegarding the blood work, the blood samplesmust be sent to an in-network laboratorySee*** The above two facts were confirmed by my insurance provider via phone on Mrach **, My insurance provider stated that the bill wasrejected in because John TMather Memorial Hospital did not send the samples to an in-network laboratory. On 02/**/14, the employees of John TMather Memorial Hospital confirmed that the blood work is covered by my insurance planThe only informationtaken from me at that time was that of my insurance card. I did not doubt that the computer database in the hospital missed the information to directemployees to send the samples to an in-network laboratory

Review: I walked in the John T. Mather Memorial Hospital on 2/**/14 seeking a blood test. I asked the cashier/registration if they took [redacted] for

the blood test. The trained employee told me yes. I gave her my insurance information, and I was not asked to pay for anything.

A few weeks later, the hospital sent me a bill. I called back and explained to the person. She understood it and accepted it. In Feb., 2015

I received the bill twice again, one of them on 2**/15. I called back to explain again to two persons. Today, I am asked to call them back again.Desired Settlement: I should not have to pay for this because I will not have my blood test performed at the hospital if I was told the truth at the

beginning.
