Review: I was referred by my preferred dentist to a now preferred provider for an implant. I had to pay (On the date of each service)all out of pocket (Total $5,000).
The date of service for this portion was on March **, 2015 for the Post of the implant $850 (paid on the day of service).
The Issue: I have been trying to get the information needed for me to submit for a reimbursement from my dental company ([redacted]) for almost one year. Dr. G[redacted] (did the work) and office staff have been delaying, lying to me, giving me the wrong Dental Code for the procedure all this time. Each time I call Dr. G[redacted] is busy call back in 3 days, in one week, etc. If I don't get it I will have to file a law suit. Need the Correct Dental Code for my reimbursementDesired Settlement: I Need the Correct (THIS TIME) Dental Procedure Code so I can file for my reimbursement from [redacted] ASAP or I have to file a law suit for all my suffering and financial hardship.
